General Requirements - Insurance Required - Material Safety Data Sheets Required - Safety Standards Plan - Spill Containment Plan The City of Wixom, City of Howell and the Village of Milford (hereafter referred to as "Municipalities") are seeking proposals for Concrete Sidewalk Replacement Services. The Concrete Sidewalk Replacement is to consist of replacing sidewalks within each of the communities as determined by the Municipalities. The general description of the Scope of Work (SOW) for the project is to provide the Municipalities with Concrete Sidewalk Replacements for direct replacement of sidewalks within the Municipality that have been identified as needing replacement by the given municipality. This work shall consist of constructing portland cement concrete sidewalk on a prepared base and subbase as shown on the plans or as directed. The prepared base shall be defined as the subgrade of the proposed sidewalk section to be constructed. The prepared base shall utilize on site excavated materials, excluding peat, muck, marl, blue or grey clay or other organic materials, or as otherwise specified. Backfilling and final restoration will be part of constructing sidewalks, unless otherwise provided. This includes seed, topsoil and straw restoration. The selected contractor shall be responsible for removing and disposing existing sidewalk/street sections. Sidewalks replaced due to tree root upheaval will require the roots to be ground down 6" below the base of the walks. The sidewalk depth is to be 4" except the sidewalks crossing the driveways and areas within 3-feet of the roadway at the crosswalks are to be 6" deep. The street replacement concrete will be 8" deep. The contractor will be required to complete one excavation in the right-of-way permit obtained from the DPW, as well as a zoning permit obtained from the Building Department, at no cost. If not already registered as a contractor, you will be required to submit the proper paperwork and fee. Before obtaining the permit(s) the contractor must also submit to the Municipality the required insurance documents. Each sidewalk form will require a pre-pour inspection by either the Building Official or Department of Public Service. Once the sidewalks are set up and the forms removed the selected contractor will be responsible for site restoration. Areas shall be raked smooth within 1" below the new walk, so that they can be easily mowed. Topsoil shall be applied in those areas as needed and then seeded with Cottage mix grass seed and straw so that those areas are completely vegetated. No roots or rocks that are larger than 2" in diameter or other foreign material shall be left exposed. ADDITIONAL SPECIFICATIONS FOR CONCRETE/SIDEWALK ROAD REPAIRS 1. Minimum patch on a concrete road will be three (3) feet wide by three (3) feet long. Lane ties will be installed at half the depth of the concrete and a minimum width of 12" and a maximum width of 16" using epoxy coated #5 rebar drilled and anchored with epoxy in accordance with MDOT standards. 2. Concrete mixtures shall be in accordance MDOT standards in regards to location of placement. (i.e. Roads/Sidewalks) work must be completed by October 31, 2024.
