The City of Washington Terrace is requesting proposals for a 3 (three) year contract for a crack seal contractor. The Contractor will be responsible for crack seal projects throughout the City. SCOPE OF WORK: Furnish and install crack seal material according to standards and specifications. May require driving the streets shown on the on the Location Map (provided by the City) to determine the quantity of sealant per street in order to fill the cracks. Crack sealing shall consist of all preparation work necessary for the proper application of the crack seal material such as cleaning, removing foreign matter, vegetation, and loose particles, and following the execution called out in the specifications. All areas and sections of roadway to be crack sealed will be marked or identified by Washington Terrace City. Contractor will be responsible for: Traffic Control (if needed) Notifying residents regarding limited driveway access Weather related requirements required for proper installation of material. To maintain adequate insurance and provide proof. JOB LOCATION: City Wide A bid bond will not be required. However, the contractor who is selected to perform the work shall promptly execute a contract if so required, secure worker's compensation and liability insurance and furnish a satisfactory and faithful performance bond in the amount of 100% of the contract price and a payment bond in the amount of 100% of the contract price. The average size of each group of projects over the past few years within the City equal to work valuing up to $20,000.00, with an estimated spending of up to $20,000.00 per year. If urgent needs arise, and work is required, prices can be negotiated between the City and the Contractor for smaller, more immediate jobs. If the cost of materials increases significantly during the life of the contract, a negotiated price between the City and the Contractor may be discussed. The contractor shall warrant and guarantee that the material and workmanship of the improvements provided for hereunder, and every part thereof, remain in good condition for a period of one (1) year after the date of completion and written acceptance by the owner and the contractor agrees to make all repairs to and maintain the improvements and every part thereof, in good condition during that time, at no cost to the owner. It is understood that the quantities, if stated, are approximate only and are for the purpose of comparing bids and the payments will only be made based on the above unit prices in the actual quantities, as determined by the owner's engineer in the completed work. Note: The contractor response time is of the essence. The City anticipates having crack seal projects completed at the first of the year, prior to May, so additional road maintenance projects can be done to the roadways if necessary. All material is guaranteed to be as specified. All work shall be completed in a workmanlike manner according to Washington Terrace City Codes. Any alteration or deviation from the above specifications involving extra costs will be executed only upon a written change order and will become an extra change over and above the estimate. This proposal is based on estimated quantities. Final billing will reflect actual units installed. Written Change Orders are not required for change in unit quantities. Note: This proposal may be withdrawn by us if not accepted within thirty (30) days. All work in the City's public right of way shall comply with Washington Terrace City's Public Work Standards and the Utah Department of Public Safety's Manual on Uniform Traffic Control Devices. Contractor shall be responsible for all Blue Stakes, in required.
