Renovation of an educational facility in Whitesburg, Kentucky. Completed plans call for the renovation of a 13,800-square-foot, two-story above grade educational facility.

Separate sealed proposals for the General Construction of Renovations to the Mountain Comprehensive Health Corporation Transitional Housing . As of July 30, 2024, construction had commenced mid-June 2024. All trades have been awarded. The previous solicitation is listed below for reference. Proposals must be accompanied by a five percent (5%) Bid Bond on Document AIA A-310. No bidder may withdraw a bid for a period of ninety (90) days after the date set for the opening of bids. The successful bidder will be required to furnish the Owner with a 100% performance and material payment bond. Proposals will be received by the Mountain Comprehensive Health Corporation (Owner) for labor, materials, equipment, and incidentals to convert the English building of the former Whitesburg High School property, located at 163 Walnut Street, Whitesburg, KY, into 22 efficiency apartments and support spaces to accommodate the countys need for transitional housing. The existing facility is two-story, approximately 13,800 SF. Work is to be performed in accordance with the plans, specifications, and addenda prepared by REB Architects, PLLC (Architect). All questions concerning plans and specifications should be directed, in writing, to the architects attention (below). All questions shall be submitted no later than eight (8) days prior to the established bid date. The information for Bidders, Bidding Requirements, Form of Bid, Form of Contract, Plans, Specifications, and Forms of Bid Bond, Performance and Payment Bond, and other contract documents may be examined at the following locations Each bid must be submitted on the prescribed form and accompanied by Form of Bid, Performance and Payment Bond, Certified Renovator Certificate. On the Form of Bid all blank spaces for bid prices must be filled in, in ink or typewritten, in both words and figures, and the foregoing Certifications must be fully completed and executed when submitted Bids must be accompanied by a certified check or bid bond, payable to the owner in an amount of not less than 5% of the bid. The award of the contract shall be made on the basis of the lowest and best bid in the interest of Mountain Comprehensive Health Corporation. No bidder may withdraw his / her bid for a period of ninety (90) days after the date set for the bid opening. The Owner reserves the right to waive informalities and irregularities and shall have the right to reject any and all bids. Successful bidder is required to furnish a Performance Bond in the amount of 100% of the contract amount and a Labor and Material Payment Bond in the amount of 100% of the contract amount. Each Demolition/Renovation project must comply with Kentucky Division of Air Pollution Control regulation 401 KAR 57: 011, this includes notification in writing to the Division of Air Pollution Control, Ten (10) days before start of project. Award of contract will be based on score of qualifications and bid price. The following appropriate bid documents shall be attached to each bid: 1. Non-Collusive Affidavit 2. Form of Bid Bond 3. List of Subcontractors 4. EPA approved certification of firm as a Certified Renovator 5. References If a bidder wishes to mail a bid, it should be mailed to Mountain Comprehensive Health Corporation., 10 East Main Street, Whitesburg, Kentucky 41858. If forwarded by mail, the sealed envelope containing the bid must be enclosed in another envelope addressed as specified Attention of bidders is particularly called to the requirements as to conditions of employment to be observed and minimum wage rates to be paid under the contract. These include Section 3, Segregated Facility, Section 109 and E.O. 11246. Further, Title VI Minority bidders are encouraged to bid. General Information: Federal Wage Rates are applicable. Conflicts of interest, gratuities, and kickbacks are defined in KRS 45A.445 and as provided for in KRS 45A.445 are absolutely prohibited. Liquidated Damages: If the contractor fails to complete the work within the time period specified in the contract, the contractor shall pay the owner $500.00 per calendar day of delay. Time of Completion: The completion time for this project will be 365 days from the Notice to Proceed.
