The bid guaranty shall be in the form of a cashier's check/official check on a solvent bank or a bid bond. If using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The check must also be scanned and uploaded with the e-bid. Company checks and personal checks will NOT be accepted. Checks shall be made payable to "City of Toledo." Liquidated Damages $400.00 per day Bid Bond : 2.5% Performance Bond 100.00% SCOPE AND SPECIFICATIONS The City of Toledo's Department of Public service is seeking bids to have approximately 2000sqft. of concrete sidewalks replaced at the Frederick Douglass Community Center, located at 1001 Indiana Ave. Toledo, Ohio 43607. Please provide a bid for the removal and replacement of the following: Remove and dispose of approximately 7000sqft. of concrete sidewalk. o Approximately 6580sqft. on parking lot level, Indiana Ave. and Hawley St. sides. Concrete to be replaced has been marked/painted with an orange X. o Approximately 420sqft. parking lot side lower level, West side of building. o Prep and repair subgrade where necessary. o Form new sidewalks. o Pour back 4" sidewalk 4000# Class C., broom finish. o Install expansion where required, caulking, sawcut, and sealer. Additional Information; o The use of any sub-contractors by the prime contractor must be submitted as part of the project paperwork packet and sub-mitted with your e-bid. o All federal/State/City codes must be maintained; and all work performed to code. o Contractor is responsible to obtain any permits as needed. o Work sites must be protected with barricades or caution tape as needed to maintain a safe environment. o Best efforts must be made to protect city property and/or grounds. o All materials, labor, and parts are to be included in this proposal. If additional repairs are found necessary during the completion of this project, they will be quoted separately o Services to be timely made, with site left clean and in safe condition. o All original invoices are to be mailed to: City of Toledo, division of Accounts, Suite 2020, One Government Center, Toledo, Ohio, 43604.
