The City of Crescent City ("City") seeks to contract with an individual, company, partnership, or other legal business organization (referred to as a "firm") to provide wastewater sludge hauling and disposal services as described in this Request for Proposals (RFP). The City owns a Class IV Wastewater Treatment Plant (WWTP), which is currently operated by a contractor. The City is looking to award a contract with an initial term of five years (FY 2024-25 through FY 2029-30). The objecve of this Scope of Work is to outline the requirements and general funconal areas believed to be necessary to successfully perform this contract. The Scope of Work is to be used as a general guide and is not intended to be a complete list of all work or requirements necessary to provide wastewater sludge hauling and disposal services. Each proposing firm must provide a concise statement demonstrating that the firm's company has the qualifications, experience, and capability to perform the requirements of this proposal. The following sections must be included: History - Provide a brief history of your company, including the number of years in business. State whether your organization is an individual proprietorship, partnership, corporation, or private nonprofit organization, and the date your company was formed or incorporated. Provide a statement as to any judgment, litigations, licensing violations, or other violations, outstanding or resolved, against your company in the past five (5) years. Background and Experience - A summary of relevant background information describing your firm's experience of major accomplishments and/or activities similar to the requirements set forth under this proposal, which demonstrates your firm's ability to provide the service described in your proposal, including the type of waste, length of haul, location of origin and destination. References - Provide a list of at least three (3) references for similar projects for the firm and each of the key personnel identified in the proposal. Include the reference's name; the name, title, and telephone number of a contact person; the dollar amount of the Agreement; the type of service provided; and the dates that services were completed. Key Personnel & Resources - A brief description of the experience and qualifications of the proposed key personnel assigned to this project and their roles. Identify resources within the firm's organization available to perform the Scope of Work, including off hour available resources in the event of an emergency. Owner Rights: The City reserves the right, at its sole discreon, to reject any or all proposals or any part thereof, or to waive any informalies or minor irregularies, technical defects or clerical errors, in a proposal, to make an award on the basis of suitability, quality of services to be supplied, their conformity with the specificaons and for the purposes for which they are required, and not confined to cost alone. The City will not be liable for any costs incurred by the firm in connecon with the preparaon and submission of this or any other proposal. False, incomplete, or nonresponsive statements in connecon with the proposal may be deemed sufficient cause for rejecon. The City shall be the sole judge in making such determinaon. The City reserves the right to cancel or disconnue the proposal process and reject any or all proposals in the event it determines that there is no longer a requirement for the furnishing of such services, funding is no longer available for the services, or it is otherwise in the best interest of the City to cancel the proposal process.
