Site work and paving for a civil project in Toledo, Ohio. Completed plans call for site work for a road / highway; water / sewer project; and for paving for a road / highway.

This project will reconstruct 0.29 miles of pavement on Collingwood Boulevard, from Central Avenue to Hackett Road. This includes removing and replacing the pavement, curb and gutter, and the drive approaches, as well as minor drainage improvements. Additionally, 1060 ft of new 12" water main will be installed. If you are e-bidding and are using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The bid guaranty may be a bid bond or a certified check or cashier's check on a solvent bank. ODOT requires a DBE Utilization plan to be submitted prior to bid opening. The following items will be required prior to awarding the contract: The apparent low bidder must submit C-92s and a DBE Affirmation form to ODOT as well. Special Notices Prevailing Wages are required for this project. It is the obligation of a contractor or subcontractor to make payment in accordance with the prevailing wage determinations. Current prevailing wage rates may be found on the Division of Purchases and Supplies site at online. A description of the work each DBE will perform. To count toward meeting a goal, each DBE firm must be certified in a NAICS code applicable to the kind of work the firm would perform on the contract. This work includes video recording along the routes of construction by the Contractor and providing of tapes and reports to City. Audio-video recording shall be of those areas affected by construction, to be replaced or restored to original condition. Limits of taping shall be 50 feet (minimum) each side of the centerline of proposed construction and the outer side of all buildings along the construction route. The haul route shall be audio-video taped along the first 2000 feet from the construction area. Any roads not normally receiving heavy truck traffic along the haul route shall also be audio-video recorded. Contractor shall notify, in writing, all residences fronting on the project and other residences affected by the project as directed by the Engineer, to schedule a time for internal video recording. Video the inside of those residences after scheduling an appointment. If residents fail to respond or do not want an internal video, so note it. Provide one copy of color audio-video tapes and a brief written summary to City of the existing condition of surface features such as drives, sidewalks, curbs,ditches, streets, (including pavement condition for full width), landscaping, trees, culverts, headwalls, retaining walls, fences, visible utilities, poles, and buildings. Of particular concern are any existing faults, fractures, defects or other imperfections exhibited in the existing surface features. This work consists of locating, furnishing, installing, and maintaining temporary sediment and erosion control Best Management Practices (BMP) for earth disturbing activity areas, developing, maintaining, amending a Storm water Pollution Prevention Plan (SWP3), performing SWP3 Inspections, filing a Co-Permittee form as required and compliance with the OEPA NPDES Permit, Clean Water Act (33 USC Section 1251 et seq.) the OWPCA, the 404 permit, the 401 WQC, the Isolated Wetland Permit, City of Toledo Requirements, specifications SCD and other related rules, laws and permits. In the event of conflict between these requirements and pollution control laws, rules, or regulations of other Federal, State, or local agencies, adhere to the more restrictive laws, rules, or regulations.
