The work includes repair and remediation to previously damaged Operations Building administration offices and all appurtenant work required as shown and specified in the bid documents. The time for completion is 45 consecutive calendar days from notice to proceed. Bid Bond, Certificate of Surety, Surety Disclosure Statement and Certification (if bid amount is greater than or equal to $800,000), Statement of Ownership Disclosure, List of Subcontractors, Disclosure of Investment Activities in Iran, Russia and Belarus Disclosure, Acknowledgement of Receipt of Notices, Revision or Addenda to Bid Document Form, Bid Proposal Forms, Non-Collusion Affidavit, Bidder's Qualification Statement and List of References, Affidavit of Permit Law and Violations, State Treasurer's List of Debarred Suspended and Disqualified Bidders, Bidder's Affidavit, Certification of Equal Employment Opportunity, Affirmative Action Affidavit, Prompt Payment Certification, Equipment Certification, Bid Document Submission Checklist, and Questionnaire are required with each Bid. Bidders shall not remove and submit the Bid pages separate from the volume of Contract Documents but shall submit their Bid bound with the complete volume of attached Contract Documents, including all pages correctly assembled and all Addenda attached. This contract or subcontract is subject to the provisions of N.J.A.C. 7:22-3,4,5, 9 and 10. In accordance with the provisions of N.J.S.A. 58:11B-26, N.J.A.C. 7:22-3.17(a)24 and 4.17(a)24, the contractor (subcontractor) shall comply with all of the provisions of N.J.A.C. 7:22-9. Bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq. and, N.J.A.C. 17:27.The Owner reserves the right to reject all Bids pursuant to N.J.S.A. 40A:1113.2 and N.J.A.C. 7:22- 9.10(a). *The list of planholders and/or interested bidders is not being tracked by the stakeholders involved with this project.
