Statement Of Work Statement Of Work Replace Roll-Up Doors General: Provide material and labor to three roll-up doors. Located at 1481 W.10th Street Indianapolis, IN 46202. Conformance Standards: All services provided under this contract must be performed in conformance with the Occupational Safety and Health Administration (OSHA), and Original Equipment Manufacturer standards and specifications. Material provided shall be installed in accordance with the American National Standards Institute (ANSI). Performance Requirement: Services shall be scheduled with the Electric Shop Supervisor Stephen Poynts. Services Required: Contractor shall provide parts and labor to replace three roll-up doors. Door #3 Qty 1 - CHI 8 x 8 Rolling Fire Door for VA 165-degree fusible links provided (standard). Closing speed regulated by centrifugal or viscous governor. Closing speed to be between 6" to 24" per second per NFPA 80. Curtain slats and hood are hot-dipped galvanized, per ASTM A653 A, with baked on epoxy primer. Guides and headplates painted black. Bottom bars to be painted black. Miller 2 wire reversing edge with single phase liftmaster firedoor operator UL / ULC 3 Hour Rated Fire Do Door #13 Qty 1 - CHI 8 x 9 Rolling Fire Door for VA 165-degree fusible links provided (standard). Miller 2 wire reversing edge with single phase liftmaster firedoor operator UL / ULC 3 Hour Rated Fire Do Warehouse Door Qty 3 - CHI 8 x 9 Insulated Rolling Steel Doors for Warehouse Replace existing warehouse doors with new CHI rolling steel doors turn Contractor Requirements: Contractor shall perform work in a professional manner and to the highest standards. Contractor shall check in with VAMC at time of arrival and checkout as defined in service hours. Contractor to complete the VA Vendor Credentialing process and have all employees working on VA property badged with VA contractor badges through Police Services before beginning any work at the VA. All contractors shall always wear VA Contractor Badges while onsite. Contractor must be licensed/certified by the State of Indiana. Contractor shall be required to follow all ICRA requirements set forth by the infection control staff. Contractor is responsible for coordinating all shutdowns with the VA and the VA Staff who will be affected by said shutdown and with the electric shop. The contractor may come to the electrical shop for guidance, but they are inherently responsible for the planning and implementation of said shutdown. Contractor is to remove all trash and debris at the completion of job. The work area is to always be kept clean. The Contractor is to ensure that all tools, equipment, and material are kept and placed in an orderly fashion to have as little impact on daily operations to the hospital, staff, and patients. Contractor shall be required to complete a VAMC Indianapolis Service Ticket when the work is completed. Contractor shall perform a function test in the presence of a VA employee designated by the Electric shop supervisor. Note: All work is expected to encounter normal conditions. Contractor to Install equipment in accordance with the manufacturers installation instructions. If anything, unusual is encountered the VA will be notified and pricing will adjust accordingly. Payment for services: Invoices will be paid in arrears monthly upon completion of the services. Each invoice must include VA purchase order number, and invoice number and the month of the service being invoiced. The invoice should be mailed to the address identified herein. Security Requirements: The Contractor s employees shall wear visible identification and company uniforms, approved by the COR, at all times and while on the premises of the VA. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section or from the Contracting Officers Representative (COR). The VA will not validate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited inside any buildings at the VA. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.
