State-certified MBE or WBE Prime Bidders are NOT REQUIRED to fill out this form. Parking Lot Repairs: Repair Sinkholes: Address and repair sinkholes around catch basins to ensure proper drainage and structural integrity of the parking lot surface. Curb Replacement: Replace missing sections of curb throughout the parking lot to delineate parking areas clearly and ensure pedestrian safety. Building Perimeter Repairs: Concrete Rams and Stairs: Repair and protect the areas where concrete ramps and stairs meet the building. Solutions should prevent water ingress and damage from winter freeze cycles. North Side Ramp Replacement: Replace the ramp by the big parking lot on the north side of the building, correcting the 4-inch sink and 3-inch pull-away from the building. West Side Repairs: Replace the ramp and slab, grade asphalt for proper drainage towards the catch basin, and ensure the asphalt is correctly pitched. Southwest Corner Ramps: Replace ramps to ensure safe and accessible entry and exit points around the building. Gate Replacement South Side Gate: Replace the existing gate to accommodate plow and emergency vehicle access, ensuring functionality and security. Add Alternates: Courtyard Drainage: Adjust asphalt pitch for improved drainage towards the catch basin, reinforce the lamp/concrete slab base, and address humps in the asphalt. South Side Ramp and Slab: Replace the ramp and concrete slab, addressing the 4-inch sink and 3-inch separation from the building for improved accessibility and safety.
