This work is estimated by the Department of Administration, Public Works Division to be less than $150,000.00. Bid must comply with the below listed requirements and utilize DAPW Form 35. Prequalification is not required. The Contractor shall provide all labor and material for a preventative maintenance service as specified herein. Contractor to provide all labor and materiel and supervision for a preventative maintenance service as specified herein for Fire Suppression Systems. a. The term of this contract is to for 730 days (2 Years) and to cover all required testing and inspections during that period as required below. b. Work shall be bid as a lump sum for the 2-year duration (730 days or 2 Years). The attached supplemental cost sheet is to be filled out and returned with the bid documents. c. Invoices permitted semi-annual upon completion of work for that period. d. Work shall include providing approved Emergency Services at an additional charge to the contract. e. There shall be a minimum of 2 inspections per year approximately180daysapart The State of Indiana may, but is not obligated to, offer this contract with the same terms and conditions except price each year for an additional three (3) years upon mutual agreement of all parties. An adjustment of the annual contract amount maybe made at the end of each year this agreement is in force.
