Responsible Contractors who have proper experience, equipment and qualifications are invited to bidonthis work. Thesefactorswill be considered in the Award of Contract and all work will be performed under the standard regulations for constructionoftheCommonwealth of Kentucky. The Lee Specialty Clinic has requested that the existing heat welded sheet flooring, vinyl compositiontilecarpettileandcove base in offices, common areas, clinical and restrooms be removed and replaced with newcommercial gradeflooringmaterial. During this process there will be multiple phases that will also require and include relocatingexistingfurnituretoa temporary on-site space or POD and replaced upon completion. The Division of Engineering and Contract Administration, Commonwealth of Kentucky, reserves theright torejectallbidsand to waive all informalities and/or technicalities where the best interest of the Commonwealthmaybeserved. Pursuant to KRS 45A.480(1)(b), an agency, department, office, or political subdivisionof theCommonwealthofKentucky shall not award a state contract to a person that is a foreign entity requiredby KRS14A.9-010toobtainacertificate of authority to t romtheSecretaryofStateunderKRS 14A.9-030 unless the person produces the certificate within fourteen (14) days of thebidor proposal opening.Therefore, foreign entities should submit a copy of their certificate with their solicitationresponse. Iftheforeignentity is not required to obtain a certificate as provided in KRS14A.9-010, the foreignentityshouldidentifytheapplicable exception in its solicitation response. Foreign entity is defined within KRS 14A.1-070. The terms of this agreement are subject to any existing or future further requirements issuedbytheCentersfor Medicare and Medicaid Services and the Cabinet for Health and Family Services regardingvaccinationrequirements, including, but not limited to, mandatory forms and processes for individualswhocometoorwork at long term care facilities.
