Site work and paving for a mixed-use development in Burlington, Wisconsin. Completed plans call for site work for a road / highway; for paving for a road / highway; sidewalk / parking lot; sidewalk / parking lot; and water / sewer project.
The work includes furnishing of all materials and equipment and performance of the following work, per the Plans, Specifications and Contract Documents (PS & CD). The Base Bid work for the above contract includes the pavement replace for 7 Alleys within the City of Burlington. The alley locations are as listed: o City Alley #1 (E Market St/Wainwright Ave & Mary St/S Pine St) o City Alley #6 (Wainwright Ave/Robert St & Edward St/S Perkins Blvd) o City Alley #11 (Robert St/Highland Ave & Edward St/S Perkins Blvd) o City Alley #23 (Orchard St/W State St & S Maple Ave/S Elmwood Ave) o City Alley #24 (Orchard St/W State St & S Oakland Ave/S Maple Ave) o City Alley #27 (W State St/Summit Ave & N Maple Ave/N Elmwood Ave) o City Alley #29 (Summit Ave/Walnut St & N Oakland Ave/N Maple Ave) Major work items and approximate quantities for all base bid work as requested include: 1,400 tons HMA Pavement, 3,600 Base Aggregate Dense 1-1/4", 1,700 CY Excavation Common, 330 LF of 12-inch RCP storm sewer, 2 storm inlet replacements, 200 SY of Concrete Driveway 6-Inch, 400 SF of Concrete Sidewalk 4-Inch, 190 LF of concrete curb and gutter replacement. These improvements also include storm inlet adjustments, sawing asphalt and concrete, erosion control, traffic control, and restoration. The project also includes a mandatory Additional Bid for City Alley #4 (Wainwright Ave/Robert St & Mary St/S Pine St). Major work items and approximate quantities as requested include: 270 tons HMA pavement, 900 tons Base Aggregate Dense 1-1/4", 450 CY Excavation Common, 370 LF 12-inch RCP storm sewer, and 3 storm manholes. These improvements also include a sanitary manhole reconstruction and external seal, replacement of concrete curb and gutter, replacement of concrete driveways and sidewalk, sawing asphalt and concrete, erosion control, traffic control, and restoration. Failure to return these documents within this prescribed time will result in forfeiture, to the Owner, of the selected Contractor's cash, Bid Bond, or cashier's check, as liquidated damages. The Bidders Qualification Statement form must be completed and submitted in a sealed envelope at the time of Bid submittal. No bids will be accepted from any Bidder who has not submitted a Bidders Qualification Statement form. These prequalification forms will be reviewed; however, updated proof of qualification may be requested by the Owner prior to acceptance of any bid. The Owner reserves the right to reject the bid of any Bidder who has been determined to be unqualified. As a part of their Bids, Bidders shall submit a full and complete list of: 1) all of their proposed Subcontractors, and 2) the class of work which each will perform. The selected Contractor shall not alter this list without the written consent of the Owner. The PS & CD require that the selected Contractor and Subcontractor(s) furnish a Certificate of Insurance. Bidders shall submit Bids on blanks prepared for that purpose and shall state prices in writing and in figures. Use the form contained in the PS & CD. Bidders should pay attention to the requirements for Bid signatures, as set forth in Bid Security. Bidders may not withdraw their Bids within the 60-day period after the date of the Bid Opening. The Owner will only consider Bids from Bidders who accompany them with bank money order, a Bid Bond, or a qualified certified check, payable to the City of Burlington, in the amount equal to 5 percent of the Bid. This requirement serves to guarantee that, if the Owner accepts the Bid, the selected Contractor will execute and deliver the proper Contract Agreement and Bonds within 15 calendar days after the Notice of Award by the Owner.
