OPWDD's Capital District DDSO is looking to obtain bids for the ADA bathroom conversion our home located at 23 Firestone Lane, Clifton Park per the attached scope of work. Service-Disabled Veteran-Owned Set Aside: No Total MWBE Participation Goals: 30% Service-Disabled Veteran-Owned Business Contracting Goal: 6% Any questions or requests for clarification regarding the information provided may be directed to psespurchasing@opwdd.ny.gov. DESCRIPTION: This project is regarding the conversion of the existing bathroom to an ADA compliant bathroom. The vendor shall provide all labor and materials necessary to complete the work in a professional manner per scope of work. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project prevailing wages, and certified payroll are to be paid. Refer to this scope of work and attached design drawings for specifications. Upon completion of the ADA bathroom, The Plant Superintendent will be notified for a final inspection. SCOPE OF WORK includes, but is not necessarily limited to the following: o Refer to the attached designs for the demolition and renovation specifications. o Any deviations from the designs must be approve through Capital District DDSO first. o Any unforeseen concerns found during this project, Capital District DDSO must be notified before work continues. o Plumbing fixtures and access schedules are included in the designs. o These attached designs are to be followed unless stated otherwise.
