The undersigned certifies that the Bid documents and addenda have been carefully examined. The undersigned declares that the scope and nature of the work to be done is understood. The RFB is also available from Attleboro City Hall, Purchasing Office, 77 Park Street, Attleboro, MA 02703. Fill out Plan Holders Form to view and download the bid documents and to get notified of any addendums. This Specification covers the quality and gradation requirements for a sub-base material combining crusher-run coarse aggregates of crushed stone and fine aggregates of natural sand or stone screenings uniformly pre-mixed witha predetermined quantity of water. In accordance with M G L. Chapter 30B, Section 5, sealed bids will be accepted at the City of Attleboro, Purchasing Department, 77 Park Street, Massachusetts 02703 until the time indicated on the advertisement for bids, for the commodities, equipment, or services designated in the specifications and will be publicly opened and read by the Purchasing Department. The clock in the office of the Purchasing Department shall be official. Bid shall be submitted on the bid form provided. No change shall be made in the phraseology of the bid or in the item or item mentioned therein. Bids must contain the name and proper address of the bidder and must be signed. If by an individual, her/him personally; if the bidder is a partnership, by the name of the partnership, followed by the signature of each general partner; and if the bidder is a corporation, by the authorized officer, whose signature must be attested to by the Clerk/Secretary of the corporation and the corporate seal affixed. Bids, which are not complete, or contain any omissions, erasures, alterations, additions, or contain irregularities of any kind, may be rejected as informal. The Contract will be awarded to "the lowest responsible and eligible bidder" for Dense Graded Crushed Stone AND Crushed Stone Materials. Contract term July 1, 2024 thru June 30, 2025 with the option to renew same pricing for two additional one year terms. The City reserves the right to request a vendor to maintain vendor records for six (6) years and to provide assistance with any future audit requirements.
