The City of Santa Maria ("City") is inviting qualified firms to submit a proposal for Construction Quality Assurance ("CQA") Services, specifically, to provide CQA services for the Santa Maria Regional Landfill ("SMRL") Cell 1 Phase II Liner Extension Project. Project description: This RFP is for the performance of CQA services during the construction of the Cell 1 Phase II Extension Project (herein after referred to as the "Project"). The selected firm will have overall responsibility for managing, coordinating, and implementing the CQA activities for the Project as well as ensuring that the quality control activities are performed in accordance with the CQA Plan and construction documents. Critical activities related to the construction, manufacture, and installation of the earthwork, geosynthetics, civil improvements, and other project components will be monitored and documented by the selected firm. The selected firm will be responsible for issuing a Final Certification Report containing CQA documentation sufficient to satisfy regulatory requirements and the requirements of this CQA Plan. Note: The selected firm may not be affiliated with the Contractor, subcontractors, suppliers, or manufacturers working on the Project. The CQA Manager and the CQA Monitor(s) will act as the representatives of the CQA Consultant. The firm will be required to perform the work further described in the below scope of services. Proposers may propose additional scope in their submitted proposal for consideration by the City. The specific terms of the agreement will be negotiated with the selected firm and the City will draft an agreement that reflects the agreed-upon terms. Scope of Services The City reserves the right to modify the following minimum scope of services based on information contained in the proposals and subsequent negotiations with the selected firm, who will be required to: 1. Review and comment on the CQA Plan and Plans, Specifications, and Cost Estimates (PS&Es) within 30 days of the execution of the agreement and issuance of a Notice to Proceed. Within the 30-day period, the Consultant will also provide a health and safety plan for field staff that addresses any potential waste and NHIS excavation. 2. Designate a CQA Manager and provide sufficient CQA Monitors to perform the duties as outlined in the CQA Plan for the Project. 3. Document that all quality assurance requirements have been addressed and satisfied and collect and maintain all related documentation. 4. Draft the Final Certification Report for review by the City. The Final Certification Report shall contain all CQA documentation sufficient to satisfy regulatory requirements and the requirements of the CQA plan. The draft Final Certification Report shall be submitted to the City within 30 days of completion of Protective Soil Cover (PSC) placement. 5. Within 14 days of receiving the City's comments on the draft, provide three hard copies and one electronic copy of Final Certification Report to the City. 6. Review and respond to all draft and Final Certification Report inquiries. 7. Supply all labor, equipment, and tools necessary to effectively perform the scope of services required by the City. Proposers should prepare a straightforward, succinct submittal that relates to the information provided herein, specifically responding to information detailed in the above Scope of Services section and this Submittal Requirements and Organization section. Any alteration, omission, addition, variance, or limitation of, from, or to a submittal may be sufficient grounds for rejection of the submittal. The City has the right to waive any minor defects or irregularities in a submittal. The City may deem a submittal nonresponsive if any document or item necessary for the proper evaluation of the submittal is incomplete, improperly executed, indefinite, ambiguous, or missing. Additionally, the City reserves the right to reject any and all submittals. The City may, for any reason, decide not to award the agreement(s) as a result of this RFP. The City reserves the right to select the successful submittal(s) and negotiate an agreement as to the scope of services, the schedule for performance, duration of the services with the firm(s) whose submittal(s) is/are most responsive to the needs of the City, and compensation for services. The City reserves the right to further negotiate any aspect of an agreement.
