As of 6/20/2024. Project has not been awarded and it's going to re-bid next spring next year. Scope of work: Culpeper County Health Dept is seeking bids to renovate and construct with new workstations located at 640 Laurel Street, Culpeper Va. 22701. I will attach a drawing of the layout of the new furniture. We will need 4 workstations, 4 - two drawer lateral file cabinets and 4- three drawer vertical file cabinets. Services to be provided: Our County will provide the demolition of the front office area and we need to purchase the modular system and have installed in its place. A site visit is mandatory for this request. Although work is preferred after normal business hours of Monday - Friday, 8:00 am-4:30 pm and weekends; we can make exceptions, if needed. Permitting- The awarded contractor must follow all permitting requirements per county guidelines. The contractor is responsible for all cost associated with permitting, if any. Walls and Flooring- Must be installed according to proper code and guidelines. Final Walk-Through- The awarded contractor must contact the project manager upon completion. Once completed, the contractor and project manager must perform a final walk-through to confirm work was completed according to the terms of the contract. Waste Removal-The awarded contractor is responsible for the removal of all waste from the premises. REQUESTED FORMS/INFORMATION (to be included with bid submittal): o Basic Design, Plans, Drawing or similar- Must indicate your plans for the project. Must show dimensions and etc. o Manufacturer's Installation Instructions (If applicable) o Warranty Period - a minimum two (2) year guarantee from date of accepted completion against defective parts or workmanship ptional* Maintenance Agreement (if applicable): list of scheduled services, terms and pricing (to include both maintenance and repairs) are requested to be included with bid submittal. *The inclusion or omission of this information will not affect award. o Certificate of Insurance - not required to be submitted with bid but will be required for contract award. o Contractor's License - A copy of the bidding firm's contractor's license. o SWaM Certification Number (if applicable)
