The Project consists of furnishing all labor, material, and equipment for rehabilitation of approximately (15) fifteen manholes on the 72-inch Biscayne Boulevard Interceptor. The work of the manhole rehabilitation shall include, but not be limited to, excavation; removal of the existing manhole corbel; furnishing and installing HDPE or fiberglass pipe as a manhole repair insert; surface preparation to install the insert repair pipe; pressure washing and repairing grouting voids; preparation of a base to install the insert repair pipe; installing a screen or barrier over the manhole opening to prevent debris from falling into the interceptor; concrete repair and restoration as needed; attaching pipe to the insert repair pipe on the interceptor; filling void in the ground with flowable fill; intercepting connecting gravity sewer systems, handling the flow during the rehab, and reestablishing the connection once the manhole is rehabbed; grouting the annular space between the repair insert pipe and the existing manhole structure; grouting any voids and sealing at the junction of the interceptor and top slab; furnishing and installing new manhole reinforced top slab; furnishing and installing a protective coating for the manhole top slab; furnishing grout and concrete with an anti-microbial additive; furnishing and installing manhole frame and cover; measuring dimensions and making cutting materials to size for the rehabilitation work; complying with all OSHA safety requirements for confined space and personal protective equipment; furnishing and installing flowable fill, if required; traffic control; legal disposal of debris; furnishing and utilizing all required safety equipment; working at low flow times when required; securing City of Miami permits; pavement restoration; and all other miscellaneous and appurtenant work for a complete manhole rehabilitation. The manholes to be repaired are located throughout the North Biscayne Bay 72-inch Interceptor system constructed over 70 years ago. Approximately (15) fifteen of these manholes shall be repaired as part of this project. The Miami Dade County Ordinance No.90-143 is applicable to this Project. The Contractor shall review the "Supplemental General Conditions" and the "Wage and Benefit Schedule" that are included within these documents. It is the intent of the Department to obtain a complete functional, and satisfactory installation under this Contract, and any items of labor, equipment or materials which may reasonably be assumed as necessary to accomplish this end shall be supplied whether or not they are specifically shown on the Plans or stated herein. The County reserves the right to assess actual damages in lieu of LDs. General Building Contractor; General Engineering; Pipe Lines; Pipelines Engineering Contractor; Underground Utility / Excavation A. License Requirements 1. At the time of the Bid and pursuant to the requirements of Section 10-3 of the Code of Miami-Dade County, Florida, and these Solicitation and Contract Documents, the Bidder must hold a valid, current, and active: a. Certificate of Competency from the County's Construction Trades Qualifying Board as a General Engineering Contractor or as a Pipeline Engineering Contractor. The Pipeline Engineering Contractor shall subcontract with a qualified contractor any work which is incidental to the pipeline specialty but is specified in the aforementioned Code as being the work of other than that of the Engineering Specialty for which certified. b. Certification, as a General Contractor or as an Underground Utility and Excavation Contractor, provided by the State of Florida Construction Industry Licensing Board, pursuant to the provisions of Section 489.115, Florida Statutes. Liquidated Damages: $1,000.00 Estimated Value: $836,600 o 7040 and 7360 RPQs with an estimated project value in excess of $700,000.00 may be assigned a Small Business Enterprise Goods (SBE-G) or Small Business Services (SBE-S) program goal. The SBE-G or SBE-S goal if applicable will be will be stipulated on the RPQ and the Invitation to Bid or in the Project's Solicitation Documents. All RPQs with an estimated project value $100,000 or above are subject to Responsible Wage Rates. The wage rate will be stipulated on the RPQ and the Invitation to Bid or in the Project's Solicitation Documents o All Projects, where price (Proposals/Bids) received are in excess of $200,000 will require the submission of the Payment and Performance Bond as required by State of Florida Statute.
