The Undersigned certifies that he (they) are the only person (persons) interested in said project and that it is made without connection with other persons submitting a proposal on the same scope of services; that the proposal is made without collusion, fraud, or reservation; that no official or employee of the Owner is directly or indirectly interested in said proposal, or any portion thereof. Provide the names and qualifications of the staff who will perform the work under this contract, including years of experience providing services required in the Scope of Work and any relevant licenses or certifications (attached additional page if needed). No sub-contractors are permitted for this work. Provide a description of projects similar in character and scope to the work required for this contract successfully completed by this Offeror during the last three (3) years. The term "completed" means accepted and payment received from the Customer. List three (3) references to include: company name, contact name, phone number, email address, and a description of the work performed for the client. References may be contacted as part of the evaluation process. Proposers may attach additional awards and letters of commendation. The vaults may contain water, which may need to be pumped out to perform the testing. The successful Offeror will be required to coordinate their testing schedule with the JCSA Project Manager, who will coordinate with the JCSA underground department to pump water out of the vaults during the week of testing. However, vendors should be aware that weather or other uncontrollable circumstances during the day(s) after they are pumped out may lead to the presence of some water in vaults. The successful Offeror shall be prepared to potentially work in, or mitigate up to 6 inches of water in the vaults. The BPA on South England is connected to an extended fire supply line servicing our hydrants. This is an extended fire service supply line, so the JCSA has a backflow prevention assembly (BPA) on the line to protect the distribution system from stagnant water concerns. The other one is at Jamestown Island and is a similar situation (a long fire supply line). It serves our hydrants and the Federal Park buildings that are located several miles away from the BPA vault site. The JCSA will provide contact information so that the Park Service can be notified by the Contractor the day of the test that the water will be temporarily off for the BPA test.
