Elevator Maintenance Service All packages must include: a signed Stockholder Disclosure Certification, a signed Non-Collusion Affidavit, a Business Registration Certificate, proof of Workmen Compensation and Employer Liability Insurance, and a signed form HUD-5369-A, Representations, Certifications and Other Statement of Bidders, and Certification of Non-Involvement Prohibited Activities in Russia or Belarus form. The Penns Grove Housing Authority is requesting the submission of proposals for a maintenance contract commencing July 1, 2024 ending June 30, 2026 for the (2) two Otis Passenger Elevators at Penn Towers. All services will be presented in accordance with Penns Grove Housing Authority Elevator Maintenance Contract specifications. The Authority reserves the right to reject any and all proposals, if it is in the best interest of the Authority to do so. The Contractor submitting the proposal that is most advantageous to the Authority with factors including but not limited to price and experience, shall be awarded the work. This contract should include emergency adjustment callback service during regular working hours. This term of the contract should be two-year intervals. Either party may terminate this agreement at the end of the year with a thirty day written notice. The term of this engagement shall be for a 24 month period beginning July 1, 2024 and ending June 30, 2026 unless 30 days written notice is provided by either party.
