Mesa Unified School District #4 ("District") is seeking the services of a qualified and experienced contractor to provide, install, and maintain a live bacteria delivery drain treatment system, according to the terms, conditions, and specifications of this solicitation. Prices submitted shall be fixed unit pricing and include all equipment, materials, labor required to maintain drain systems at various Mesa school locations. Biological Drain Treatment System and Maintenance It is the intent of Mesa Unified School District #4 (District) to secure a fully qualified and experienced contractor, to provide, install, and maintain a live vegetative bacteria delivery drain treatment system, according to the terms, conditions, and specifications of this solicitation. Prices submitted shall be fixed unit pricing and include all equipment, materials, and labor required to install and maintain drain systems at various Mesa school locations, to include, but not limited to, rental of pump, chemicals, and maintenance of unit. The District is responsible to pay sales tax on materials and supplies. List tax rate as a separate line item. The goal of this contract is to provide preventative drain system treatment that provides potential reduction of sewer line blockages and helps reduce the amount of grease trap pumping through the school year. Installation of equipment will be required at four (4) school kitchen locations within the District at the beginning of the contract. Any new equipment to be installed shall be completed no more than thirty (30) days from award of contract. The contractors must be able to work more than eight (8) hours per day and weekends if required to complete the job. All equipment provided shall remain the property of the contractor throughout the term of the agreement. Contractor shall be responsible for all service, cleaning, and any routine repair of the system. Contactor shall be responsible for removal of all equipment at the end of the contract. Results of this solicitation will be available online after an award is made. Mesa Unified School District is the largest school district in Arizona. The District is located primarily in Mesa, Arizona and is comprised of approximately 200 square miles. The District maintains over 80 educational facilities within the District as well as approximately 12 administrative facilities. The scope of this service shall include five (5) school locations in the District. This quote is intended to be a three-year agreement for services estimated at $30,000 per year. There is no expressed or guaranteed quantity and usage shall be determined solely by District need. Question Submission Deadline: May 24, 2024, 12:00am. (Example: If awarded on July 15, 2024, the contract would still only run through June 30, 2025). If all conditions are met during this period, this contract can be extended, if funding is available, for up to four additional one-year contracts or any portion thereof. However, no contract exists unless and until a purchase order is issued each fiscal year. Basis of Award. A. An Award will be made to the Responsible Offeror whose Offer is determined to be the most advantageous to District based on the requirements of the Solicitation and can meet the district's schedule. B. Multiple Awards. The District's preference is for a contract under this quote to be awarded to a single vendor. However, the District reserves the right to make a multiple award to more than one offeror. The award will be limited to the least number of offerors that the District determines is necessary to meet the needs of the District. C. A response to the Solicitation is an offer to contract with District based upon the terms, conditions, scope of work/services, and specifications contained in the Solicitation. An Offer does not become a contract unless and until District accepts it. A contract is formed when the District Representative signs the Award document on behalf of District. No work may commence or products be delivered until District has issued a Purchase Order to Contractor. D. Sales tax will not be a consideration in the award of the quote. E. The District shall be the sole judge in determining the award, and its decision shall be final. The District reserves the right to reject or accept any quotes.
