The successful bidder shall be required to comply with the provisions of the New Jersey Prevailing Wage Act, Chapter 150 of the Laws of 1963, effective January 1, 1964. All contractors are required to comply with the requirements of N.J.S.A. 52:32-44 (Business Registration), N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27 et seq. (Contract Compliance and Equal Employment Opportunities in Public Contracts). Submission by corporations, partnerships and limited liability companies shall include a completed Disclosure of Ownership form (N.J.S.A. 52:25- 24.2). Submissions shall also include a completed Non-Collusion Affidavit. All requirements of the State of New Jersey Worker Health and Safety Act (N.J.A.C. 12:110 et seq.) as amended and the United States Occupational Safety and Health Act (OSHA) (29 CFR 1910), as amended with regard to worker and jobsite safety. A copy of the Surety and Bid Bond must be uploaded with the bid documents and, the originals must be submitted to the Township of Hamilton, Department of Budget and Purchasing, 2090 Greenwood Avenue, Hamilton, NJ 08609 by the date, time and place stated in this ad. SCOPE OF WORK - The contractor shall thoroughly review all plans and specifications prior to construction. Plans and specifications must be referred to simultaneously to fully comprehend what is to be constructed. This project consists of milling and paving Roberts Ave between Nottingham Way and East State St. The roadway is to be milled up to 3" in depth (3,400 S.Y.) to be checked for base failure and repaired (up to 60 S.Y.) if needed and overlaid at least 2" compacted thickness of HMA 12.5M64 (475 Tons). Inlets shall be repaired where directed and the existing headers and grates shall be replaced with new Eco Type headers and grates that meet ADA compliance. Utility manholes, water and gas valves are to be reset to grade. This project includes the construction of concrete sidewalks (approximately 600 S.F.) and concrete curbs (approximately 170 L.F.) as directed; replacement and/or construction of 4 accessible ramps to conform with Township specifications and the requirements of the Americans with Disabilities Act; excavation and grading to proper elevation as directed; construction of pavement repair along new curb; topsoil fertilize and seed all disturbed areas. All disturbed areas shall be reshaped and graded to insure positive drainage and match existing topography and grades.
