The City of Holly Springs (City) invites and will receive Bid Proposals on the forms attached hereto, all information on which must be appropriately completed. One (1) completed Bid Packet, marked "Original", as well as, six (6) photocopies of the completed Bid Packet shall be included in each Proposer's envelope. The envelopes containing the Proposals shall be sealed, plainly marked " MDOT WATERLINE RELOCATION ALONG SR178" on the outside, and addressed as follows: Office of City Clerk P.O. Drawer 520 160 South Memphis Street Holly Springs, MS 38635 The envelope shall also bear on the outside the name of the Proposer, as well as, address of Proposer, Bid Proposal Title "MDOT WATERLINE RELOCATION ALONG SR178", and RFP Closing Date. The project consists of, but is not limited to, the following: x Install 1,640 linear feet 8-inch diameter PVC potable waterline. x Install/replace four (4) water service connections inclusive of water meters with valve box, copper service line and service tap to new waterline. x Cut, plug and abandon "existing" waterline and remove "old" water meters and valve boxes. x Install one (1) 8" tapping sleeve & valve (TS&V) x Install two (2) 8" gate valves. x Install five (5) fire hydrants and assemblies. x Restore all affected areas after installation activities. x Maintain Traffic Control during construction per MDOT standards.
