The project Scope of Work is included as Exhibit A. Proposal pricing is to be based on the Scope of Work and includes the cost of bond(s), insurance, sales tax, and every other item of expense incident to the contract. a. Carpet Pad Residential FHA/HUD/UM approved 72 Type 3 urethane foam, (d) bonded specifying density to be 6 lbs. per cubic foot - 5% and a minimum of 1/2 " thickness with moisture barrier. Identification: At least every 10 lineal feet, one cushion surface shall contain a reference to FHA Bulletin on carpet pad and the name of manufacturer or a designated and registered identification number and product classification by type and class. New pad musts be installed when installing new carpet. b. Reducers Adapters & Transition: Rubber Reducers Adapters & Transition are to be used. The transition strips material is "Snap Track" a snap down transition with the appropriate rubber insert between the vinyl, carpet, tile, and wood transitions. Transition strips shall be provided and installed by Contractor with each new flooring installation. (Color to be selected by the Facilities Maintenance Manager or designee to match flooring installed). The lump sum figure includes the cost of bond(s), insurance, sales tax, and every other item of expense incident to the contract. The work will be completed within 365 calendar days from the date of commencement of work. as set forth SCOPE OF WORK: See Exhibit "A" dated May 20, 2024; See Exhibit "A.3.A" CADA Residential Buildings; See Exhibit "A.3.B" CADA Commercial Properties; and see Exhibit "B" TERMS AND CONDITIONS. The term of this Contract shall commence on July 1, 2024, and end midnight June 30, 2025. Authority may extend the term of this Contract under the same terms and conditions and at the same rates for an additional period of one year by giving written notice to Contractor at least sixty (60) days prior to expiration. The Agreement is subject to the Authority's Board of Directors providing funds for said services in its annual budgeting, to continue on an annual basis for up to a period of two (2) years. Owner Rights: The right to reject any and all bids or to waive any informality in any bid received is reserved by CADA. The award shall be made to the lowest responsible and responsive bidder, based on the lump sum amount provided on the Bid Proposal Form, provided that all specified bidding requirements have been met. Once the contract has been awarded to a bidder, the successful bidder then has ten (10) calendar days to provide CADA with a signed original contract along with all other required documents. If the above deadline is not adhered to, CADA reserves right to void the award, and award the contract to the next lowest bidder. Contractor agrees to provide Authority with Certificates of Insurance as outlined in Exhibit B, Section 39, prior to execution of this contract. Notwithstanding the provisions of Item 26 of Exhibit B -- Terms and Conditions -- Contractor is not required to provide either the Payment or Performance Bonds referenced. Project Has been Rebid
