PROJECT DESCRIPTION: The Industrial Drive Improvements Project is approximately 2.121 miles long beginning 1000-ft north of the S. Main St. intersection northerly to the curve at Falling Springs Rd. The project involves HMA pavement patching, 2-ft widening, milling and overlay, 2-ft wide aggregate shoulders, culvert removal and replacement, fence removal and replacement, and other miscellaneous items. Interested bidders should submit the following forms in a sealed envelope labeled Industrial Drive Improvements.a.Local Public Agency Formal Contract Proposal (BLR 12200) b.Schedule of Prices (BLR 12201) c.Proposal Bid Bond (BLR 12230), if applicable The Village of Dupo reserves the right to waive technicalities and to reject any or all proposals. Bidders must be prequalified with the Illinois Department of Transportation. The prequalification process is two-fold. To satisfy the first requirement and to be issued a proposal, prospective bidders must furnish a certified or photostatic copy of their currently dated Certificate of Eligibility to the Local Agencys Representative indicating sufficient financial rating for 100% of the project and work rating categories to cover 50% of the project. If a prospective bidder downloads proposal documents from the SIBA website, they are only eligible to submit a proposal after they have furnished their Certificate of Eligibility to the Local Agencys Representative for review. This must be done in advance of the letting. If emailed, the subject line must include Industrial Drive Improvements Certificate of Eligibility. It is the responsibility of the bidder to obtain confirmation of email delivery. The second prequalification requirement is that the 2 apparent as read low bidders must file within 24 hours after the letting an Affidavit of Availability (Form BC57), showing all uncompleted contracts awarded to them and all low bids pending award for Federal, State, County, Municipal and private work. The form shall be filed with the Local Agency Representative above. An overall Business Enterprise Program (BEP) utilization goal of 8% ($114,216 of the grant funds) has been determined for this project with 5% of the grant dollars going to minorityowned business enterprises (MBEs) and 3% of the grant dollars going to womenowned business enterprises (WBEs). The bidder must demonstrate that either the UP goal has been met or that it has made good faith efforts to do so. The apparent low bidder shall submit a completed BEP Utilization Plan on the forms provided within five calendar days after the date of the bid opening. To meet the five calendar day requirement, the bidder may send the BEP Utilization Plan electronically by scanning and sending to the following email address: rdell996@ gmail.com. The subject line must include Industrial Drive Improvements BEP Utilization Plan. The BEP Utilization Plan should be sent as one PDF file rather than multiple files and emails. It is the responsibility of the bidder to obtain confirmation of email delivery. Alternatively, the BEP Utilization Plan may be sent to the Village of Dupo by certified mail or delivery service within the five calendar day period. The BEP Utilization Plan may be submitted to: 107 N.
