Clay County is requesting quotes from qualified contractors to replace the well at the following location: 6293 Alliance Avenue Keystone Heights, FL 32656 Question Submission Deadline:June 13, 2024, 4:30pm The Clay County Board of County Commissioners are requesting Quotes from licensed Contractors to abandon an existing well and install a new well at the address listed below: 6293 Alliance Avenue Keystone Heights, FL 32656 The maximum amount that can be spent on any one (1) residence is twenty-five thousand dollars ($25,000) per the Program. If any additional services are requested from the homeowner, the Contractor will be required to notify the County Project Manager or designee. No additional services shall be performed until the Contractor receives written authorization from Clay County. 2.2. Federally Funded This is a Federally Funded Project under Community Development Block Grant Entitlement Program funding by the Department of Housing and Urban Development (HUD). The Contractor's performance under the Contract shall be subject to 2 C.F.R. Part 200 - Uniform Admirative Requirements, Cost Principles and Audit Requirements for Federal Awards. The Federal Government has transitioned from the use of the DUNS Number (i.e., an identifier issued by Dun and Bradstreet ) to the Unique Entity Identifier (UEI) as the primary means of entity identification for Federal awards government-wide. UEIs are required in accordance with 2 CFR Part 25, and the transition from DUNS to UEI has resulted in the UEI being issued by the Federal Government in SAM.gov. The Contractor is required to be familiar with and agrees to abide by all applicable State and Federal, or municipal emergency, safety and health codes, laws, rules, regulations or ordinances, including but not limited to all FEMA and the U.S. Department of Housing and Urban Development (HUD) provisions. Additionally, the Contractor agrees to comply with all DEO guidelines. 2.3. Scope of Work Work includes, but is not limited to obtaining and providing all materials, labor, and equipment necessary to complete the project and the following tasks: General Conditions Mobilize Permitting Installation of safety fencing around work site. Abandonment of Existing Well Abandon and grout fill existing well. New Well Construct the new well at a minimum of ten (10) feet and maximum of fifteen (15) feet from the existing well. Drill new 4-inch well, using PVC well casing, drilled up to 340 feet. Install new 1 Horse Power (HP) submersible pump. Install one hundred twenty (120) feet, one (1) inch galvanized drop pipe. Install submersible wire. Install one (1) super well seal. Install one (1) thirty-two (32) gallon capacity bladder tanks. Tie-in and reconnect to existing water and electric (existing connections are located at the existing well). The Contractor is responsible for disconnection and reconnection of attached electrical components. After installation, test the well pump system to ensure proper functioning. Test calibration to achieve the appropriate water pressure and flow rate, twenty (20) gallons per minute. Current Conditions The two (2) inch existing well was installed in the 1970s. The well will not supply an adequate flow rate to the residence. The current flow rate is approximately two (2) to three (3) gallons per minute.
