This Submission will consist of separate sealed proposals for the following: Construction Management Services All proposals must include the following: Proposal Pricing Form (required or disqualified) Notarized Disclosure Statement - Familial Relationship (required or disqualified) Contractor Information Form Iran Economic Sanctions Act Form Certification Form Further, the Proposer will execute the contract, provide the required insurance certificate(s) and file the required bonds within (10) days after notice of award of Contract but prior to Work commencing. The successful Proposer may also be required to furnish a Performance Bond and Labor and Material Payment Bond in the amount of 100% of any contract price on any awarded projects over $50,000 through this RFP. Please refer to the RFP evaluation criteria for information on determination of award(s). Ann Arbor Public Schools reserves the right to reject any or all proposals received and to waive any informalities and irregularities in the submissions.
