Roosevelt School - Phase 1 Improvements This Project is the Renovation of the Middletown Township Community Center Site and Includes the Following: Removal and Replacement of Asphalt Paving, Installation of New Asphalt Paving, Installation of Asphalt Sealcoat, Installation of Storm Sewer and Stormwater Management Facilities, Expansion of an Existing Detention Basin, Bulk Grading, Construction of the Pad for a Tot Lot (Installation of Playground Equipment is Not Included in This Contract), Installation of Fence, Installation of Pavement Striping and Traffic Control Signage, Tree Removal, Concrete Curb, Depressed Curb, Mountable Curb, Concrete Sidewalk and Walkway, Concrete Surfaces and Handicapped Accessible Ramps, Limestone Courtyard Surfaces, Installation of Roof Leader System and Underdrain, Erosion and Sedimentation Controls, Placement of Topsoil, Finish Grading, Seedings and Stabilization. Must Be Accompanied by a Noncollusion Affidavit, Statement of Bidder's Qualifications Form, Proposal Form, and a Certified, Bank Cashier's, or Treasurer's Check or Bid Bond With Corporate Surety Satisfactory to the Township in an Amount Not Less Than Ten Percent (L0%) of the Total Bid Price, Payable to the Township. Attention of Bidders is Called to All of the Requirements Contained in This Bid Packet, Particularly to Insurance Requirements, Workmen's Compensation Requirement, Various Equal Opportunity Provisions, and the Requirement for a Payment Bond and Performance Bond for 100% of the Contract Price, With a Corporate Surety Approved by the Township. The Contractor Who Has Been Successfully Awarded the Contract Shall Complete Work Within Ninety (90) Calendar Days From the Notice to Proceed.
