Questions will need to be submitted by July 25, 2024 at 2:00 p.m. and Responses will be issued by us via an Addendum no later than July 29, 2024 at 2:00 p.m. All questions must be posted through Electronically. The City is seeking a contractor to replace a paved over storm manhole frame and cover. The manhole is located in West Main Street (CT Route #847) at the intersection of Sperry Road, Waterbury, CT. The existing manhole is approximately 6" below grade. The contractor will be responsible all traffic control measures, i.e. uniformed police officers or certified flaggers, cones, barrels, barriers, signs etc..., that is necessary. All work is to conform to DOT and City of Waterbury standards. Scope of Work: The Successful Bidder (a/k/a, the "Contractor") shall provide all labor, materials, tools, equipment and permits necessary to complete the project, which includes the following: 1. Obtain all permits from the City of Waterbury and the State of Connecticut Department of Transportation. 2. Set up traffic patterns as required using cones, barrels, construction signs etc. See attached Technical Specification 9.71R Maintenance and Protection of Traffic and West Main Street Sperry Street Detour plan. 3. Cut/Core a neat circular line in the pavement around the existing manhole frame with a shell manhole cutter (circular cutter). A jack hammer or pavement saw cut is not acceptable. During construction, the Contractor is to keep debris from entering the inside of the manhole. If debris does enter the inside of the manhole, it must be removed. 4. The material-gravel, pavement and concrete collar (if there) shall be removed down to six (6) inches below the frame. 5. Install masonry (brick or a concrete grade ring) on the manhole to the necessary height to set the manhole frame so that it matches the existing grade and pitch of the existing pavement. Repair/modify the existing manhole as needed to accept the new frame and cover. 6. Set the new manhole frame and cover (to be supplied by the City) on a bed of mortar. Remove any asphalt and debris from the existing cover. 7. Backfill, compact and pave per Conn DOT specifications. 3 inches of HMA Super pave 0.5 Traffic Level 2 placed in two equal lifts. 6 inches HMA Super pave 1.0 Traffic Level 2 placed in two equal lifts. 10 inches Processed Aggregate Base Material compacted in 5" lifts. Compacted Granular Fill base compacted in 8" lifts. Tack the sides and hot joint seal the perimeter of the cut with a 6" wide continuous strip of RS-1 (Rubberized Sealant) or approved equal. 8. The Contractor is to coordinate all work with other contractors that are working in the general area. The City of Waterbury shall receive written notice of cancellation from the Contractor at least thirty (30) calendar days prior to the date of actual cancellation, regardless of the reason for such cancellation. The Contractor must supply replacement/renewal certificates at least 30 days prior to the expiration of the policy(ies). Said certificates shall contain a provision that coverage afforded under the policies shall not be cancelled or reduced for any reasons unless notice of than thirty (30) calendar days has been mailed to the City's Using Agency and a copy to the City's Office of Corporation Counsel, 235 Grand Street, Waterbury, CT 06702. No later than thirty (30) calendar days after receipt by the Contractor, the Contractor shall deliver to the City a copy of the Contractor's insurance policies and endorsements and riders.
