In no event shall failure to inspect the site constitute grounds for a claim after contract award. Site Visit will be held: Date: July 30, 2024 Time: 1000 hours Central Time Point of Contact: Jeremy Kasprak [Jeremy.Kasprakusda.gov or (715)-401-1358] Address: Northern Research Station, 5985 County Hwy K Rhinelander, WI 54501 Questions and Answers to date attached. ******************************************************************* USDA Forest Service Northern Research Station (NRS) Rhinelander is requesting offers to furnish all labor, supervision, equipment, supplies, and transportation necessary as per the attached Statement of Work to provide services for Abatement & Removal of Old Tile Flooring and Luxury Vinyl Installation at the following location: U.S. Forest Service Northern Research Station 5985 Highway K Rhinelander, WI 54501 This solicitation is issued as a Request for Quote (RFQ). FAR Part 13, Simplified Acquisitions Procedures, will be used. A contractor may be eliminated from consideration without further exchanges if technical and/or pricing proposals are not among those Offerors considered most advantageous to the Government. This solicitation is 100% set-aside for total small businesses associated with NAICS 238330, size standard $19M. Period of Performance for the resultant contract will be starting within 30 days and completion within 90 days upon contract award in accordance with all terms, conditions, provisions, and specifications of the solicitation. This solicitation will result in a Firm-Fixed Price contract to a single offeror determined to be responsive and responsible. Offeror must be registered in the System for Award Management (SAM) website: https://sam.gov in order to submit an offer. The solicitation document and incorporated provisions and clauses are those in effect through the date of posting. It is the Offeror's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at https://www.acquisition.gov/content/part-52-solicitation-provisions-and-contract-clauses. MUST provide a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with your offer. The proposed contract will be awarded to the Offeror that represents the Lowest Price Technically Acceptable (technical capability, and price) to the Government. HOW TO SUBMIT OFFER: Offers will only be accepted via e-mail. It is the Offeror's responsibility to verify offer was received. Offer received after the due date and time shall not be considered. Amendments, if any, to this solicitation will be posted through Contract Opportunities on https://sam.gov. Paper copies of the amendments will NOT be individually mailed. By registering to receive notifications on https://sam.gov, offerors will be notified by email of any new amendments that have been issued. No other notification of amendments will be provided. Potential Offerors are advised that they are responsible for obtaining and acknowledging any amendments to the solicitation. FAILURE to acknowledge an amendment may result in your offer being considered unacceptable. The Government intends to select one contractor for this acquisition. However, the Government reserves the right not to award a contract at all, depending on the quality of the offers and prices submitted and the availability of funds. Rejection of Unreasonable Offers: The Government may reject any offer that is evaluated to be unreasonable in terms of program commitments, including contract terms and conditions, or unreasonably high or low in cost when compared to Government estimates, such that the offer is deemed to reflect an inherent lack of competence or failure to comprehend the complexity and risks of the program. Each response must clearly indicate the capability of the Offeror to meet all specifications and requirements. When submitting an offer it is very important that all offerors understand it is their responsibility to ensure they meet all requirements of the RFQ, not just those requirements stated in the specification such as terms and conditions, representations and certifications. Offers will ONLY be accepted via e-mail. All other offers will be rejected. Description of Requirement This project will entail removing tile flooring, glue, and areas of needed fill floor self-leveler compound. There is roughly 3690 square feet of old tile flooring to be removed. Out 3690 square feet, 2490 square feet test hot for asbestos either in tile or the glue. 3 offices need luxury Vinyl plank flooring installed and doorways will have transition strip. Floors shall be interlocking vinyl plank, low maintenance, 20-year commercial warranty. A Luxury Vinyl Plank will be installed approximately 48 inches by 6 inches and must be interlocking with the appropriate underlayment sheeting as required by the manufacturer. A new 6-inch cover base will be installed. Selection of the flooring colors shall be approved by the COR and Contracting Officer of the Northern Research Station 13, Rhinelander WI.
