The service of a licensed contractor with asbestos remediation experience is requested to properly perform demolition to include, without limitation: Dismantle, remove and dispose of the fire damaged SFR on this property, including the accessory structures on the north side of the property, removal of the partial wire fence on the west side of the property, and any foundations and walkways and/or driveways on the property. Removal of all junk, trash, debris, and materials on lot. Perform clean-up, leaving the demolition site clear of any debris. Furnish any required clean fill dirt and regrade the jobsite leaving the lot vacant and ready to build. NESHAP regulations must be followed. A Notice of Demolition (Form 62- 257.900) must be furnished to FDEP and a copy must be provided to the City prior to commencing demolition. Unless otherwise specified, the Contractor shall: 1. Obtain a demolition permit from the City of Winter Haven Building Department. 2. Disconnect all utility services under City's supervision before demolition. 3. Complete and submit a Notice of Demolition (Form 62-257.900) to FDEP. A copy must be provided to the City prior to commencing demolition. 4. Demolish, remove, and dispose of the structures, foundation, steps, and/or walkways. 5. Remove all materials from the demolition site in accordance with federal, state and local regulations. Including any asbestos materials as listed on the attached Asbestos Survey Report. 6. Remove and dispose of appliances and other items that may contain refrigerants, including refrigerators, freezers, dehumidifiers, portable air conditioners, and central air conditioners, in accordance with Code of Federal Regulations, Title 40, Part 82. 7. Remove and legally dispose of mercury-containing materials including fluorescent, high-pressure sodium, mercury vapor, metal halide bulbs, and thermostats containing a liquid filled capsule. 8. Perform site clearing and grading. Final grading shall be level.
