Notice is hereby given that quotes will be received at the Los Angeles County Development Authority (LACDA), until Wednesday, August 14, 2024, at 3:00 p.m. for the "HUD Lead-Hazard Reduction Program: Lead-Based Paint Hazard Remediation and Construction Services (LHR-004)" for the properties located at: 1. 411 N. Chester Avenue Unit 1, Compton, CA 90221 2. 411 N. Chester Avenue Unit 2, Compton, CA 90221 3. 411 N. Chester Avenue Unit 6, Compton, CA 90221 4. 411 N. Chester Avenue Unit 7, Compton, CA 90221 5. 2718 Orchard Place Unit A, South Gate, CA 90280 6. 2718 Orchard Place Unit B, South Gate, CA 90280 7. 2718 Orchard Place Unit C, South Gate, CA 90280 8. 2718 Orchard Place Unit E, South Gate, CA 90280 9. 6024 Fostoria Street, Bell Gardens, CA 90201 10.6024 1/4 Fostoria Street, Bell Gardens, CA 90201 11.6026 Fostoria Street, Bell Gardens, CA 90201 12.6028 Fostoria Street, Bell Gardens, CA 90201 13.6034 Fostoria Street, Bell Gardens, CA 90201 This will include furnishing all labor, material and equipment, and performing all work necessary and incidental in accordance with Appendix B - "Scope of Work" A Request for Quotes (RFQ) solicitation package containing all submission requirements may be downloaded from the LACDA's website at www.lacda.org, scroll to and click on "Vendors," click on "View Open Solicitations," and under the "Title" column, select the solicitation of interest, then click on "Download Documents Below," and follow the instructions. Quotes shall be submitted on the attached Quote Sheet with required forms, proof of contractor's license, CDPH and EPA certification, and insurance coverage with a cover letter. All quotes shall be labeled "HUD Lead-Hazard Reduction Program-Lead-Based Paint Hazard Remediation and Construction Services LHR-004. Contractors are solely responsible for ensuring their Quote is received by the LACDA in accordance with the solicitation requirements by the above stated due date and time. The LACDA shall not be responsible for any late or delays by mail or common carriers, or by transmission errors or delays, or mistaken delivery including electronic mail. Deliveries made before the stated due date and time but to the wrong department/unit, will be considered nonresponsive, unless re-delivery is made to the correct department/unit before the stated due date and time. Quotes received after the due date and/or time will be considered late and will not be considered for award. In submitting a quote, each contractor thereby agrees, that if awarded a contract, they shall execute a standard LACDA contract and forms, a sample of which is attached as Appendix A - Sample Contract to this Request for Quotes. The Contract is between the LACDA and Contractor. The LACDA shall not award a contract to, or be obligated to execute a contract with, any contractor who refuses to execute said standard LACDA contract. Contractors are advised that this RFQ is an informal solicitation and should not be construed as engaging in a formal competitive bidding pursuant to any statute, ordinance, rule or regulation. The LACDA reserves the right to reject any and all quotes. This RFQ is not a contract or commitment of any kind. The LACDA is not liable for costs incurred in the preparation of the quotes submitted by contractors. It reserves the right to issue supplementary information or guidelines related to this RFQ. Notwithstanding any other provisions herein, the LACDA reserves the right in its sole discretion to waive minor technical deficiencies in the quotes. In accordance with the Civil Rights Act of 1964, Americans with Disabilities Act of 1990, Age Discrimination Act of 1975 and Section 504 of the Rehabilitation Act of 1973, it is the policy of the LACDA to assure equal opportunity to all persons, in the award and performance of any contract, without regard to race, color, sex, religion, national origin, ancestry, age, marital status, or disability. CONTRACT REQUIREMENTS: Insurance Requirements (please refer to Section 9 - Insurance, of the sample Agreement) The proper documentation showing evidence of all required coverage is required to be submitted and approved by the LACDA within two weeks from the date Contractor was notified of being the apparent lowest responsive and responsible bidder. Failure by the Contractor to meet this requirement will prohibit the award of a contract. General Liability: shall include products/completed/on-going operations coverage, an additional insured endorsement providing coverage on a primary and noncontributory basis and a waiver of subrogation. ($2,000,000.) Workers' Compensation: shall be Statutory and include Employer's Liability, and a waiver of subrogation. ($1,000,000.) Automobile Insurance: coverage shall include owned, hired, non-owned, OR any auto and a waiver of subrogation. ($1,000,000.) Crime Insurance: the LACDA and their Agents shall be named as loss payee on such policy (Employee Theft Coverage $25,000, Forgery Coverage 25,000, Client Coverage $25,000) Pollution Liability Insurance and/or Asbestos Pollution Liability and/or Errors & Omissions: Applicable to the work being performed including coverage for bodily injury, personal injury, death, property damages, and environmental damage with limits of not less than the following: General Aggregate ($2,000,000), Each Occurrence ($1,000,000). Bonds: Contractor License Surety Bond Licensing and Certifications: The Bidder must possess a current and valid B-General Building Contractor License issued by the California State License Board. The Bidder or approved Subcontractor must possess all required certifications in compliance with the requirements set forth in the California Department of Public Health's (CDPH) Lead Related Construction Program. License must be current and valid for the duration of the contract. Current CPDH Certificates for all Supervisors and Workers overseeing and performing lead-based paint hazard remediation activities. EPA "Lead-Safe" Certification issued by the United States Environmental Protection Agency. The certification must be current and valid for the duration of contract. Penalty - (please refer to Section 12 - Penalty, of the sample Agreement) Penalty: If the Work is not performed in accordance with the terms of this Agreement and in compliance with the Notice to Proceed, the LACDA is entitled to impose the Penalty of five hundred ($500) dollars per day. The Work to be performed under this Construction Contract shall commence within ten (10) days after a Notice to Proceed is received by the Contractor, or on the date specified in the Notice, and shall be completed within thirty (30) calendar days following the required commencement date, or on the date specified in the Notice by the LACDA. Completion Period: 30-Calander Days from date noted in the Notice to Proceed Non-Completion Penalty: $500.00 per day Greater Avenues for Independence (GAIN) and Skills and Training to Achieve
