Request for Proposals San Miguel County ("County") requests statements of qualifications and proposals from qualified firms and individuals to provide culvert excavation and replacement services on FS626, also known as County road A63/North Trout Lake Road, in southeastern San Miguel County, near Trout Lake. The County seeks qualified contractors or firms to provide a comprehensive bid to excavate, remove, and install a new culvert and backfill, compact, and grade to the existing level. Background San Miguel County is a County Road and Bridge Department that maintains this road as a part of our Schedule "A" agreement with the Forest Service (USFS). USFS will be purchasing the replacement culvert and San Miguel County is responsible for the removal and installation of it. Due to our schedules to maintain other county roads, we are seeking contractor services to complete this project on or before October 15, 2024. There are no approvals or permitting that will be necessary from the contractor or firm, as the County has already secured those permissions with the USFS. Scope of Work and Deliverables Contractor is asked to provide qualifications and prior experience with other projects involving deep excavations. A timeline and cost breakdown of labor and materials to excavate the dirt over the failed culvert, which may be staged in the roadway. Removed the failed culvert, divert the stream that flows through it in order to bed the new culvert in the original location and then to replace the dirt, compacted in 12" lifts back up to the road surface. See Attachment B for the picture of the existing culvert including approximate depths and lengths. See Attachment C for the full scope of work. Inquiries can be emailed to the attention of Mike Bordogna, San Miguel County Manager- Mikeb@sanmiguelcountyco.gov using the subject line: "Trout Lake Dirt Work" The deadline for inquiries is Friday, August 16, 2024, at 5:00 PM Mountain Standard Time. Proposal Requirements The maximum page count for the proposal is 8 (eight) pages submitted electronically. Resumes can be added to the proposal as part of the proposal's appendix not applicable to the maximum page count of eight (8) pages electronically. Proposals must include clear descriptions of the firm's capabilities and experience in performing the requirements of the RFP, including the following: Transmittal Letter. This letter should outline the Contractor's ability to provide the experience and scope of services requested. Please provide a profile of the firm and identify the primary project contact for the proposal. Staff Experience. Include a description of each key staff assigned to a typical project. Identify any sub-consultants proposed to serve on the project as well. This should include identification of experience working on similar projects for similar clients. Proposed Project Approach and Estimated Timeline. Explain the consultant's understanding of the project, itemize expected project activities identified by task, and include an estimated project timeline for the project steps. Sample Projects. Include up to Three (3) sample projects which includes similar tasks and deliverables to what the County seeks. Include a brief description of each project and owner contact information for reference checks. Cost Proposal. Include a fee proposal outlining hourly rates of firm participants as well as cost estimates per task as identified in the Approach and Timeline section. Exceptions. Please include a statement regarding exceptions to the County's sample contract included in Attachment A. If no exceptions are included, the County expects the Consultant will be able to sign the County's contract and provide the required insurance and indemnification. Signatory Requirements. The Transmittal Letter must be signed by the contractor's representative who is empowered with signatory authority for the company.
