As of September 13, 2024. Project was awarded to Fahrner Asphalt Sealers in the amount of $58,527.50. Upon entering a contract with the Village of Bellevue, insurance certificates and bonds representing the following must be presented by the party submitting successful proposal. The Contractor shall furnish the VILLAGE with certificates of insurance as evidence of the required coverages. The certificates are to be on the form provided by the VILLAGE and must be received and approved by the VILLAGE before work commences. The VILLAGE reserves the right to require the Contractor to furnish complete, certified copies of all required insurance policies, at any time. The form of certificate is included in the Invitation for Bids for this project. The VILLAGE'S failure to notice or notify the Contractor of any coverage deficiencies that may be apparent in the documents submitted to the VILLAGE shall not relieve the Contractor of responsibility to provide coverage required in this contract. The Contractor shall provide an Umbrella Liability Policy of: TWO MILLION DOLLARS ($2,000,000). The cancellation clause on all certificates of insurance shall read as follows: "Should any of the abovedescribed policies be cancelled before the expirations date thereof, the issuing company will mail 30 days written notice to the certificate holder named herein." (No modification to this clause will be accepted.) VILLAGE OF BELLEVUE 2024 Chip Sealing Project CONTRACT CS-24 VILLAGE OF BELLEVUE CHIP SEALING SPECIFICATIONS Pavement Chip Sealing The chip sealing process will consist of a polymer emulsified asphalt blend and a 100 % fractured 3/8 washed limestone chip or 3/8 dark colored granite chip which shall not exceed 3/8" in size. Both asphalt and aggregate will be applied to one half of the roadway at a time to allow for continuous flow of traffic. The polymer asphalt emulsion shall be applied at 170 degrees and be evenly distributed by a spray distributor. Road surface shall be cleaned by a self-propelled broom within seven (7) days of application and any necessary cold mix or hot mix patching installed prior to chip sealing. The finished chip seal will be rolled and set in place with a duo packer roller. Cold mix asphalt for wedge patching will be produced using a SC-800 asphalt and a crushed stone and washed sand aggregate mixture with a top size not exceeding 1/2 " Bid shall give a price estimate as an alternate to have village staff complete any necessary cold or hot mix patching installed prior to chip sealing. Pavement Sweeping Contractor shall be responsible for sweeping and removing all loose limestone or granite chip material no sooner than (5) business days from application of the chip seal surface. Disposal of loose material shall be the responsibility of the Contractor. Fog Sealing Application Upon completion of the chip sealing process, Contractor shall Fog Seal roadways selected by Bellevue using a single layer of emulsified asphalt. Fog sealing shall not be applied any sooner than (5) days and no later than (14) days from chip seal application. Road surface shall be free and clear of loose chip and debris prior to fog application. Concrete driveways shall be protected so as not to overspray concrete in the sealing process. Traffic Control The awarded contractor is responsible for following traffic control in accordance with MUTCD guidelines. All costs for traffic control will be at the expense of the contractor. Hours of Operations (Work Hours) In accordance with the Village of Bellevue Ordinance 188-4. The crack fill operations must take place Monday - Friday 7a-3:00p. There shall be no construction-related activity beyond the hours listed above which disturbs the peace and quiet of any neighborhood. The Village of Bellevue will be accepting its Chip Sealing bids for 2024. We are hereby requesting you to submit a proposal to this office. The Village reserves the right to reject any or all bids, waive any in informality in the bids received and to accept any proposal which they deem most favorable to the interest of the Village.
