The City's ST-3 form can be found on the Certificate of Tax Exemption page of the City of Minneapolis website. If a lump sum bid for materials and/or equipment includes labor and all incidentals, the bidder is responsible for all applicable sales tax on taxable items required in the performance of the bid and should be included in the total amount of the bid.
