Site work and paving for a sidewalk / parking lot in Mesa, Arizona. Completed plans call for site work for a sidewalk / parking lot; and for paving for a sidewalk / parking lot.

The intent is to establish a three (3) year term contract for concrete installation, repair, and utility adjustment services on an as-needed, where-needed basis for the City. Work will include, but will not be limited to: concrete removal, installation, repair, and other related services as required and requested by the City. 2.SCOPE OF WORK: Work under this Agreement shall include, but is not limited to: 2.1 Concrete installation and repair services: concrete removal, installation, repair, and other related services. Contractor shall provide all labor, materials, equipment, and tools required to perform concrete installation and repair services under this contract. Materials shall include, but not be limited to: concrete pavers, asphalt cold patch, and aggregate base coarse material. Questions concerning this Solicitation should be submitted in writing through the City of Mesa's Procurement Services Website Vendor Self Service portal . SCOPE OF WORK: Work under this Agreement shall include, but is not limited to: 2.1 Concrete installation and repair services: concrete removal, installation, repair, and other related services. Contractor shall provide all labor, materials, equipment, and tools required to perform concrete installation and repair services under this contract. Materials shall include, but not be limited to: concrete pavers, asphalt cold patch, and aggregate base coarse material. 2.2 Utility Adjustment services: minor concrete work and the adjustment of gas and water valves, survey monuments, sewer cleanout, and manholes associated with utility cut repairs and/or failed utility frames and other related services. Contractor shall supply all concrete material. City of Mesa (City) will supply all manhole and valve hardware, frames, and lids. 2.3 Contractor must dedicate one (1) complete crew to the City Monday through Thursday year-round. Contractor must be able to stand up an additional crew within thirty (30) days upon request from a City representative. 2.4 Contractor shall operate all trucks, trailers, and other equipment in compliance with all applicable federal, state, and local rules and regulations. Equipment shall be in good working condition.
