The City of Marshfield is soliciting proposals for construction services for City owned facilities in general and in particular our facility located at 915 S. Marshall. Vendors are required to submit written proposals that present the vendor's qualifications and understanding of the work to be performed. The vendor's proposal should be prepared simply and economically and should provide all the information it considers pertinent to its qualifications for the Specifications listed herein. Emphasis should be placed on completeness of services offered and clarity of content. Proposal Content - A completed proposal must contain the following: Proposal Form & Signature Page - the proposal form and signature page must be completed and signed by an individual authorized to bind the contractor. All proposals submitted without such proposal form and signature page may be deemed non- responsive. References - Proposals shall include a list of three (3) references including name, address, phone number and contact person. It is preferred that references are those of Southwest Missouri contacts. The City reserves the right to contact references other than, and/or in addition to, those furnished by a contractor. Proposal Period - Proposal prices are to be firm for thirty (30) days. Proposal Award - It is the intent of the City to accept the lowest responsible proposal, provided it has been submitted in accordance with the proposal documents. If a proposal is selected it will be the most advantageous regarding price, quality of service, the Contractor's qualifications, and capabilities to provide the specified services, and other factors which the City of Marshfield may consider. The City reserves the right to accept or reject any or all proposals and to waive irregularities therein. Proposals will be approved by the Board of Aldermen at a regular scheduled meeting. Term and Renewal - The term of the Contract shall be for one (1) year with the option to renew up to three (3) years, unless earlier terminated. The Contract may be terminated by either party with a ninety (90) day written notice. The Contract may be terminated by either party with or without cause in less than ninety (90) days by mutual agreement or in the event of substantial failure to perform in accordance with the terms set forth in the Contract. Basis of Payment - Payment will be made to the contractor within 30 days upon receiving the contractor's monthly invoice and after approval by the Board of Aldermen. The invoice shall state the date the services were performed and amount and which facility. Additional services provided outside the specifications included in this request will be billed via a separate invoice and described by the services provided and the date they were provided. The specifications outline the requirements for the construction services for the facility located at 915 S. Marshall Street and typical type of services requested for other facilities. Construction Requested: Remove existing slider type window including existing exterior siding to allow for new window, modify existing header and frame wall to accommodate new 40"x 49" drive-thru window station (Provided by Owner). Patch drywall, trim with stained wood and rosettes to match existing. Reinstall exterior siding as required. Install new 12" wide by 16" tall payment drop-box (Provided by Owner) through the wall. Remove existing exterior siding, frame for box, reinstall siding and patch drywall, trim with stained wood and rosettes to match existing. Remove and discard existing small drive-up canopy. Install new 6'x14' single slope canopy with metal roof (Color by Owner), install soffit and facia of new canopy to match existing. Install rough-in electrical for canopy lights (provided by others). Install 9 lineal feet of box base cabinets and 5.5 lineal feet of box upper cabinets including shelves, doors, drawers, end panels and toe kick (Provided by Owner). Install 2 lineal feet of box base cabinets and 6 lineal feet of upper box cabinets including shelves, doors, and toe kick (Provided by Owner). Provide and install a 24" x 69" laminate counter with 4" integral back splash. Install 2 lineal feet of box base cabinets including shelves, drawers, end panels, and toe kick (Provided by Owner). Provide and install a 24" x 68" laminate counter with 4" integral back splash. Construct new 48" x 68" wood platform with CCA lumber over concrete floor and 3/4 " plywood top. Install six (6) 8' wide x 4' tall solar shades (Provided by Owner). Remove existing toilet fixture in restroom to be reused and remove existing ceramic floor tile and grout and clean concrete floor for new flooring. Install new mini split unit on exterior wall of Information Technology (IT) Room (Unit provided by Owner). Install the following hardware (Provided by Owner) as indicated below per room. location. LOCATION: TYPE: IT Room Keyed Lever type with closer Janitor Keyed Lever type with closer Restroom Privacy Lever type with closer Mechanical Room Lever type with closer Copy Room Lever type Men's Restroom Privacy Lever type with closer Women's Restroom Privacy Lever type with closer Storage Room South Door Lever type with closer and sweep Storage Room North Door Lever type with closer and sweep Council Chambers Double Door Keyed Lever type with double closer and double sweep Council Chambers Single Door Keyed Lever type with closer and sweep Front Office Keyed Lever type and closer The undersigned hereby submits the following proposal for the cost of construction services for the City of Marshfield building located at 915 S. Marshall in particularly: Remove existing window and install new drive-thru window. Install new payment drop-box. Remove existing canopy and install new 6'x14' canopy Install 9' of base and 5.5' of upper cabinets. Install 2' of base and laminate countertop. Install 2' of base and laminate countertop. Construct new wood platform. Install solar shades. Remove existing ceramic floor tile. Install new mini split unit. Install door hardware.
