PROJECT: Sinkhole Remediation-Storm Line Repair Your Firm is invited to submit your Statement of Qualifications to become eligible for a possible interview for professional services related to the sinkhole remediation in the Henryville School parking lot. All interested applicants can contact the superintendent for a site meeting and scope of work details. Please provide the following document pertaining to the project: Requirements for Statement of Qualifications Preliminary Scope of Work Schedule of Activities Cost Estimate All contractors will need to complete the score of work on Time and Material basis with open book pricing. All responses will be reviewed for the selection of firms to be interviewed. Requirements for Statement of Qualifications Your Statement of Qualifications should include the following information: Name, address and brief description of firm. A one-page narrative as to firm's interest, particular abilities and qualifications related to this project. Describe other projects designed by this firm and key personnel pertinent to this project. Include reference contact information. Provided examples of knowledge, expertise and/or experience with other related work. Firm's proximity to the work area. Additional information as required by the owner. Evaluation Criteria: Proposal Evaluation criteria shall include: Specialized experience or technical expertise of the organization and its personnel in connection with the scope of services to be provided and complexity of the project. Past record of performance on contracts, including quality of work, timeline and cost control, including financing. Familiarity of this type of project or problems applicable to the project. General Conditions: The school corporation reserves the right to reject any and/or all Statements of Qualifications at its sole discretion. Certificate of Insurance will need to be presented to the school corporation before any onsite can take place. All contractors will need to have a background check on file before being allowed onsite.
