The project site is located at 533-535 Silver Lake Scotch Town Road, Middletown, New York. The intent of the limited hazardous materials survey was to identify suspect ACM, PCBcontaining caulk, mold and universal waste/miscellaneous suspect hazardous materialscontaining items that are located within designated areas of Hostel No. 9766 and may be impacted during a proposed kitchen renovation project. The limited hazardous materials survey was conducted for the subject areas, as directed by Michael Tyler, representing DASNY. The subject areas were occupied and operational at the time of the sampling event. All questions pertaining to this Project must be received no later than the final date to submit questions. FINAL DATE TO SUBMIT QUESTIONS: Tuesday, September 24, 2024 LOCATIONS: Silverlake Road SOIRA - 533-535 Silver Lake Scotchtown Rd, Middletown, NY 10941 Grange Rd IRA - 26 Grange Rd, Otisville, NY 10963 CR Number: 2113379 The Office for People With Developmental Disabilities (OPWDD) Contract Management Unit, Design and Construction, on behalf of the Hudson Valley DDSOO, is soliciting sealed bids from responsible and qualified contractors to perform required work for Installation of New Fire Alarm Systems at 533-535 Silver Lake Scotchtown Rd, Middletown, NY 10941 and 26 Grange Rd, Otisville, NY 10963. Contractors who are interested in submitting a Bid for this project should carefully examine and be familiar with the Drawings and Specifications, and the Project Manual including all Bidding, Contracting, and General Requirement Specification Documents. The Selected Low Bidder will be responsible for providing documentation of mandatory pre-award submittal requirements, including but not limited to: Vendor Responsibility Questionnaire (CCA-2), references and experience, list of proposed sub-contractors, detailed estimate, and proof of required insurances. MWBE GOALS: _0_% SDVOB GOALS: _0_% Each bid must be prepared and submitted in accordance with the Instructions to Bidders and must be accompanied by a bid security i.e. certified check, bank check, or bid bond in the amount of $8,200.00 The Office for People With Developmental Disabilities reserves the right to reject any or all bids.
