Contractor agrees to provide to the Department of California Highway Patrol (CHP) Clear Lake Area office, the services described herein: furnish all supplies, materials, tools, equipment, labor, personnel, and supervision; pay all taxes, insurance, bonds, license and permit fees, and all other direct and indirect costs necessary to provide Parking Lot Repair Services as set forth in this Agreement. A. License Requirement/Certification: Contractor must possess a valid C-12, Earthwork and Paving Contractor, California Contractor's State License Board Specialty License. B. Contractor and subcontractor(s) shall be registered with the Department of Labor Relations for public works projects pursuant to Labor Code section 1725.5. C. Contractor shall provide a Payment Bond equal to 100 percent of the value of the Agreement. Contractor shall complete State of California Standard 807, in duplicate, and submit prior to execution of Agreement. 2. The services shall be performed at: Department of California Highway Patrol Clear Lake Area office 5700 Live Oak Drive Kelseyville, CA 95452 3. The services shall be provided during: Work shall be performed on weekends, evenings and/or holiday weekends. Office is open for business on weekdays, Monday through Friday, 8:00 - 5:00, unless other arrangements are approved by the CHP on-site contact.
