This is a service / maintenance or supply contract in High Point, North Carolina. Contact the soliciting agency for additional information.
The Contractor will mow, edge and trim each site no less than once per week, or as frequently as directed by the HPHA, from the Notice to Proceed in the first year and from April 1st or when new growth first occurs in subsequent year(s) until mid-November or until growth ceases; leaves will be raked, collected and removed once weekly from October 15th until December 31st unless otherwise notified; and debris will be removed from each site after each activity and said debris will be disposed of off site at a suitable landfill or other approved location to keep the properties clean and attractive. The contractor will provide services for other items including, but not limited to: mulch, dead heading plants, fertilizer, weeding, aeration and lime application. All proposals must contain the following completed forms: 1. Responder's Statement (Attachment A) 2. Profile of Firm (Attachment B) 3. Cost Proposal (Attachment C) 4. Other requested information on the Proposal Evaluation Criteria form (Attachment D) 5. Financial Statement or Audit 6. Section 3 and MWBE Certifications 7. Change Order Acknowledgement form 8. Compliance with Executive Order No. 11246 9. References/Experience 10. Non-collusive Affidavit 11. Estimated Project Work Force - Prime/Sub Contractor 12. Certifications and Representations of Offerors Non-Construction Contract (form HUD-5369-C) 13. E-Verify Affidavit 14. Personnel Certification Question Deadline 09/26/2024 at 10:00 AM ET All questions must be sent to HPHA Estimated Initial Contract Period: Start Date 12/01/2024 End Date 11/30/2025 It is very important that each company sends to HPHA all of the required paperwork. An addendum will be issued by 5:00 p.m. on September 30, 2024. Any proposal received later than the specified time, whether delivered in person or mailed, will be disqualified. Faxed, Emailed, or other unsealed submissions will not be accepted. Contract for work under this RFP will obligate the Contractor to not discriminate in employment practices. HPHA encourages minority owned, women owned and Section 3 businesses to participate. The HPHA reserves the right to reject any and all bids/proposals and to waive minor informalities in the bidding/RFP process.
