Proposals will not be accepted by email or facsimile. Proposals must be submitted electronically through the County Electronic Procurement Network (ePro). All Proposers must register with the ePro system prior to the date and time to submit the proposal or they will be disqualified. Contract Term Services to be provided under this Request for Proposals (RFP) are outlined under Section V, Scope of Work. The Contract period will be for a maximum of five years beginning on approximately November 18, 2024 and ending on November 17, 2029 Location of Services Location where Services are to be provided, completed and managed is at 400 N. Pepper Ave., Colton, CA 92324. The Proposer must include in Proposal, Attachment E - Cost, all transportation, lodging, and per diem costs sufficient to pay its personnel and travel to the aforementioned locations. Deadline for Submission of Questions September 30, 2024 @4:00PM (PST) Questions regarding the contents of this RFP must be submitted in writing in ePro on or before the Deadline for Submission of Questions. All questions will be answered, and both the question and answer will be posted as an Addendum to the RFP in ePro. A mandatory site walk will be held for this RFP at 10:00 a.m. PST on September 23, 2024. Proposers who will attend the site walk must RSVP their attendance with the contact below by 12:00 p.m. PST on September 20, 2024. Pepper Ave. Colton, CA 92324 Attendance at the site walk is mandatory. No Proposal will be accepted from any Proposer who fails to attend the Site Walk. SCOPE OF WORK A. BACKGROUND INFORMATION ARMC is a 456-bed, state-of-the-art, acute care facility, embracing advanced technology in all patient care and support areas. ARMC offers the latest in-patient care by providing a full range of inpatient and outpatient services, four off campus community health centers, behavioral health inpatient services and numerous specialty services. ARMC is fully accredited by the Centers for Medicare and Medicaid Services (CMS) and The Joint Commission. ARMC is accredited as an American College of Surgeons Level I Trauma Center, with the second busiest emergency department in the State of California. ARMC provides emergency care for approximately 130,460 County residents, 113,769 admissions and approximately 254,000 outpatient visits annually. B. PROJECT DESCRIPTION 1. Project goal and objectives - Provide monitoring, preventative maintenance and unexpected repairs to ARMC's underground storage tanks. 2. Deliverables/Requirements: a. Maintain five (5) 20,000-gallon Diesel storage tanks (Red Dye Diesel). b. Maintain four (4) 15,000-gallon Propane storage tanks. c. Maintain one (1) 1,000-gallon Hazardous Waste storage tanks. d. All-inclusive maintenance of all underground diesel, propane (LPG) and hazardous materials storage tanks (UST) systems includes: i. Monthly monitoring and compliance management. ii. Preventative maintenance of fuel and environmental monitoring systems. iii. Provide written monthly reports regarding UST systems. iv. Provide hazardous waste removal, when needed. v. Creation and implantation of in-house industrial hygiene, safety plans and for UST on behalf of ARMC. vi. Annual preparation and electronic submission of Cal EPA Hazardous materials plan for UST on behalf of ARMC. vii. Provide annual anode testing. viii. Respond within four (4) hours of emergency calls, including Veeder-Root alarms/emergencies. ix. Respond within 48 hours to inquiries and/or audits from regulatory agencies. x. Provide training to the Central Plant staff (all shifts). e. Required Testing: i. Annual - Monitor certification and Spill Bucket Testing: 1. Five (5) 20,000-gallon Diesel Tanks - Red Dye Diesel 2. One (1) 1,000-gallon Waste Tank ii. Annual - Cathodic Protection Testing: 1. Four (4) 15,000-gallon Propane (LPG) Tanks iii. Annual - Preparation and electronic submission of CAL Environmental Protection Agency (EPA) Hazardous Materials Plan. iv. Triennial - Secondary Containment Testing with OPI Drop Tube Testing: 1. Five (5) 20,000-gallon Diesel Tanks - Red Dye Diesel 2. One (1) 1,000-gallon Waste Tank f. Inspect the entire system to find possible early issues. g. Make unexpected repairs, not exceeding $60,000, to USTs and related equipment. h. Monitor equipment for unexpected problems and failures. i. Ensure that all of ARMC's USTs and monitoring equipment are kept in compliance and in proper working order. j. Contractor shall use a hazardous waste manifest and a transporter registered with the California Department of Toxic Substances Control (DTSC) to transport the waste to an appropriately permitted hazardous waste management facility operating under a DTSC permit. The transporter must have completed the California Highway Patrol's Basic Inspection of Terminals Program with a satisfactory rating. The Contractor shall submit to County documentation of proper disposal of the hazardous waste within 5 business days of transport. The contractor shall immediately inform County of any spill or discharge of hazardous waste or potentially hazardous waste verbally and in writing. k. The Contractor shall submit to the County monthly service reports of site visits, in areas, no later than 15 calendar days. Additionally, supervisors' quality inspections should be included in the monthly reports. l. The Contractor shall ensure spill prevention to prevent the impact to stormwater and surface water through Spill Prevention Containment and Countermeasures (SPCC) m. The Contractor shall submit to the County a Quality Control Plan (QCP) for approval 15 calendar days after award of the contract. The plan will cover all work performed by the Contractor and all subcontractors. This QCP shall include: i. A description of the Contractor's QCP ii. A chart showing the Quality Control (QC) organizational structure. iii. Names and qualifications, in resume format, of each person in the QC organization. iv. The duties responsibilities and authorities of each person in the QC organization. v. A listing of all subcontractors that will be employed by the Contractor, a description of the services their firms will provide and who will be responsible for providing QC service on their services. vi. Procedures to identify, record, track and complete rework items. vii. Documentation procedures including proposed report formats. The procedures must include provisions for recording the results of inspections and for recording corrective action taken. viii. Procedures used to record client complaints and follow up inspection process. ix. Procedures for identifying and documenting the final inspection process. x. Procedures that describe the process used to determine overall client satisfaction and the quality of services provided by the Contractor. n. The Contractor shall supply the County with an Emergency Notification Plan that includes emergency telephone numbers with an emergency on-site response time of four (4) hours or less. The Contractor shall provide emergency services including, but not limited to, monitoring and adjustment of the UST systems on a 24-hour-a-day, 7 day-a-week basis. PROPOSAL SUBMISSION A. General 1. All interested and qualified Proposers are invited to submit a Proposal for consideration. Submission of a Proposal indicates that the Proposer has read and understands the entire RFP, including all appendixes, attachments, exhibits, schedules, and addenda (as applicable) and that all concerns regarding the RFP have been resolved. 2. Proposals must be received by the designated date and time. All Proposers must register with the ePro system prior to the date and time to receive the proposal or they will be disqualified. Late or incomplete proposals will not be accepted. Electronic response must be submitted through the San Bernardino County Electronic Procurement Network (ePro) System-related issues in ePro shall be directed to the Purchasing Department at (909) 387-2060. For procurement questions involving ePro, please contact the RFP Contact identified in Section I, Paragraph B - RFP Contact. 3. The Proposer acknowledges that its electronic signature is legally binding. Submittals in ePro will be opened from the system's encrypted lock box after the deadline and evaluated as stated in this RFP. 4. Proposals can be withdrawn at any time prior to the scheduled Deadline for Proposals. 5. Proposals must be submitted in the format described below. Proposals are to be prepared in such a way as to provide a straightforward, concise description of capabilities to satisfy the requirements of this RFP. 6. Proposals must be completed in all respects as required in this section. A Proposal may not be considered if it is conditional or incomplete.
