The sites of construction are along Grangemouth Drive and through Soldiers Memorial Park between the Beach House Parking Lot and Waverly Road Parking Lot, all within the City of La Porte, Indiana. Bids must be on the Bid Form contained in the Specifications. Each bidder must also complete a Form 96 (as prescribed by the State Board of Accounts); an executed non-collusion affidavit; signed copies of addenda (if issued); a completely executed Affidavit of Compliance with Indiana Immigration Law and Certification of Not Being Engaged in Investment Activities in Iran and Debarment; Affidavit of Compliance with City of La Porte Responsible and Responsive Bidder Requirements; and satisfactory bid security as set forth in the Contract Documents. All bids must be accompanied by a certified check or bid bond in the amount of 5% of the Bid. No bidder may withdraw his bid within 60 days after the actual date of the opening thereof.
