All work contained on plans and specifications as prepared by Davis Stokes Collaborative, PC (Architect) including the construction of an approximate 11,000 SF expansion to the existing Orthopedic Institute facility at 27 Southern Pointe Parkway in Hattiesburg, MS containing six(6) operating rooms and support space and the renovation of select areas within the existing facility. Bids shall be submitted in duplicate only upon the blank bid forms provided with the project manual and must be accompanied by bid security in the form of a certified check or acceptable bid bond in an amount equal to at least five percent (5%) of the base bid. Said security to be forfeited, as liquidated damages, not penalty, by a bidder who may be awarded the contract, but who fails to carry out the terms of the proposal, execute the contract and post the required performance and payment bonds in the form and amount, and within the time specified. The bid bond, or the certified check, if used in lieu thereof, shall be payable unconditionally to Forrest County General Hospital. The Owner reserves the right to reject any and all bids and to waive informalities. Therefore, observation of the auger-cast pile installation and preparation of accurate construction records will be a very critical aspect. These services should be performed by a geotechnical engineer and/or geotechnical engineering technician who is knowledgeable concerning auger-cast pile construction methods, material properties and design concepts. A prework conference with all involved parties to discuss the work plans and the observation procedures is strongly recommended. The foundation subcontractor should be required to submit a statement of qualifications during bidding that lists previously completed projects similar to this project, and they should also submit a work plan to the owner prior to start of construction that demonstrates an understanding of the work required and also shows that proper equipment will be available to perform the work.
