2025 Water Service Line Repairs Site Address: 2201 George Flagg Parkway Description: The Contractor shall provide all necessary permits, labor, machinery, tools, and specified materials to identify and document existing service line materials; adjust, repair, and/or replace curb stops, curb boxes, service lines, and appurtenances for DMWW customers. This includes sitework, restoration, and cleanup during the calendar year in accordance with the specifications provided. The initial contract term shall be from January 1, 2025, to December 31, 2025. The contract may be reviewed and renewed on a yearly basis for up to two (2) additional years, subject to mutual agreement by both parties. The work completed under this contract will address correcting deficiencies in curb stops, curb boxes, service lines, and appurtenances. This work is the responsibility of the individual property owner ("Customer"). Work under this contract will normally occur when the property owner refuses to complete the work required by DMWW under its rules governing customer service but may also occur at the request of the customer. Estimated quantities provided in the proposal are not guaranteed. SUMMARY OF WORK A. The general scope of Work is to repair or replace components of private water services that private property owners are required to repair or replace by Des Moines Water Works (DMWW). The Work shall be completed and accounted for solely through work orders issued by DMWW. B. Contractor shall be responsible for providing all permits, labor, machinery, tools, and specified materials necessary for adjusting, repairing and/or replacing curb stops, curb boxes, service lines, and appurtenances, and performing associated work including site work, restoration, and cleanup, as outlined in the Proposal and these Specifications. C. Contractor shall be responsible for identification and documentation of existing water service line material using the Stop Box Repair Form included in this Contract.
