Miscellaneous asphalt and concrete work- New Construction o 6" Base, 2" Asphalt Binder, 2" Asphalt Surface o 6" Base, 1.5" Asphalt Binder, 1.5" Asphalt Surface Stamped Asphalt o 6" Base, 2" Asphalt Binder, 2" Asphalt Surface o 6" Base, 1.5" Asphalt Binder, 1.5" Asphalt Surface Mill and Pave o 2" Mill & Pave, Tack, 2" Asphalt Surface o 1.5" Mill & Pave, Tack, 1.5" Asphalt Surface Concrete o Rough grading for slabs under the shelters, protecting tree trunks and root zones (If applicable) o 4,500 psi concrete with City standard Fiber Mesh o Subgrade compacted to 95% Proctor Density o Grading path to pitch to the nearest low spot o Sidewalks and Ramps 100 SQFT minimum Contractor's Suggested minimum o Typical sizes of slabs 28' x 28' or 34' x 34' o Smooth, broom finish o Tooled control Joints o Troweled Edges New Construction o 4" Concrete o 4" Concrete, 4" Base o 4" Concrete, 6" Base o 4" Concrete, 8" Base Each bid must be accompanied by a Bid Bond or Certified Check, for not less than ten percent (10%) of the total amount of the bid payable to the Clerk/Treasurer, of the City of Gary as security for execution of the contract under the terms embodied in the Contract Documents. In addition, the Bid must be accompanied by the following Documents at the time of the Bid Opening: a. Fully completed and executed State of Indiana Form 96, (latest addition). b. A fully completed and executed Non-Collusion Affidavit of Prime Bidder, a copy of which is included as part of Form 96. c. Bid Bond. d. Bidder s current Affirmative Action Program e. Required MWBE Subcontractor Commitment Form. Bids must be submitted on the Bid form included in these documents. The form must be fully completed and executed. No bids will be considered which are not based on these Contract Documents. The bidder awarded the contract will be required to furnish a separate Performance Bond and Payment Bond, each in the amount of one-hundred percent (100%) of the contract price before commencing work. The Performance Bond and the Payment Bond shall remain in effect throughout the entire construction period. Bid security of all except the two lowest responsive and responsible bidders will be returned within fourteen days after the bid opening. The remaining bid securities will be returned after the successful bidder has executed the Contract, posted bond and provided required certificates of insurance. If the contract is not executed, any bidder whose bid security has been retained may, after sixty days following the opening of bids, request return of his bid security, provided he has not been notified of acceptance of his bid. Any bid may be withdrawn prior to the scheduled time of the opening of bids or authorized postponement thereof. However, no bid may be withdrawn for sixty days after the scheduled time for opening, unless agreed to by the City of Gary. The City of Gary or representatives appointed by the City will make the award of the contract. In determining who the lowest responsive and responsible Bidder is, the City of Gary will consider the bid and all other relevant facts or matters mentioned in the Contract Documents which the City of Gary may legally consider in determining the lowest responsive and responsible bidder. The City of Gary reserves the right to reject any or all bids. Without limiting the generality of the foregoing, any bid which is incomplete, obscure, conditional, not on the prescribed form, has erasures or corrections in prices, includes additions or deductions not called for or is not accompanied by the required bid security and supporting documents and information may be rejected. However, the City of Gary reserves the right to waive any informalities or minor defects on the basis of the city s best interests. The City of Gary may make any investigation deemed necessary to determine the ability of a bidder to perform the work. Bidders shall furnish any information and data requested by the City of Gary to facilitate such investigations. The City of Gary reserves the right to reject any bid if the evidence submitted by, or investigation of, a bidder fails to satisfy the City of Gary as to the bidder s qualification to carry out the contract obligations. Total Budget of up to $3,000,000 for miscellaneous asphalt and concrete work.
