Site work and paving for a sidewalk / parking lot in Anderson, Indiana. Completed plans call for site work for a sidewalk / parking lot; and for paving for a sidewalk / parking lot.
For the: Reconstruction of Curbs & Sidewalks on West 16th Street from Arrow Avenue to Madison Avenue In accordance with the drawings and specifications prepared by the City of Anderson Department of City Engineering. Interpretation or explanation of Contract Documents will not be made by Local Public Agency (Owner). All such inquiries shall be made to the Architect or Engineer. If any person submitting a bid proposal for work is in doubt as to the true meaning of any part of Contract Documents, he may submit to the Architect or Engineer a written request for an interpretation. Any interpretation of such Contract Documents will be made to addendum issued. Any Bids received after the designated time will be returned unopened. Bid proposals shall consist of the following properly and completely executed documents: 1.Bid shall be submitted on the Itemized Bid Form. 2.Bid shall be submitted on General Bid Form 96 as prescribed by the Indiana State Board of Accounts. 3.Each bid proposal shall include a financial statement, a statement of experience, proposed plan or plans for performing the public work, and the equipment available for the performance of the public work. 4.Each bid proposal shall include a properly executed non-collusion affidavit as required by the statues of the State of Indiana. 5.Bid Security: Pursuant to Indiana Statute I.C. 36-1-9-6, a bidder shall file a certified check or bid bond with each bid, in an amount equal to ten percent (10%) of the total bid proposal, as evidence of good faith that the successful bidder will execute within ten (10) consecutive calendar days from acceptance of a bid proposal, a contract as set out in the specification. 6.City of Anderson Employment Eligibility Verification. 7.Bidders written plan for a program to test Bidders employees for drugs in accordance with I.C. 4-13-18. 8.City of Anderson certification of Non-Segregated Facilities. 9.City of Anderson Non-Discrimination Clause. 10.City of Anderson Company-Wide Workforce Breakdown Form. 11.City of Anderson Project Workforce Breakdown Form. 12.City of Anderson Affirmative Action Steps. 13.Ineligible Contractors Form. 14.Conflict of Interest Questionnaire. 15.*Section 3 Intent to Comply (Be advised of the updated regulations effective 07/01/2021). 16.*Section 3 Plan (All updated Section 3 documents are included in bid packet). 17.Section 3 Business Concern Self-Certification Form. The Instructions to Bidders contained in the Contract Documents are by this reference, made a part hereof and all Bidders shall be deemed advised of provisions thereof and conditions of the Contract, Specifications and Drawings for this project. Civil Rights: The successful Bidder will be required to comply with Title VI and Title VII of the Civil Rights Act of 1964 and Title VIII of the Civil Rights Act of 1968. Wage Rates: Attention is called to the fact that no less than minimum salaries, wages and applicable other benefits, as established by the United States Department of Labor and included within the contract documents must be paid on this project. Performance Bond and Labor Material Payment Bonds: The successful Bidder will be required to provide a 100% Performance Bond and Labor and Material Payment Bond in accordance with said provisions included within the Contract Documents. Build America, Buy America Act (BABAA) - Attention is called to the requirements instituted by the Bipartisan Infrastructure Law of 2021 mandating domestic preference that all iron and steel, manufactured products, and construction materials are produced in the United States. Manufacturers Certification documentation must be provided by a Manufacturer, certifying that the items provided by Manufacturer meet the domestic preference requirements by BABAA. *Other Federal Requirements: The successful Bidder will be required to provide a properly executed Section 3 Worker and Targeted Section 3 Work Certification; List of all businesses on Project (Contractor and Subcontractor Activity, HUD form 2516); Project Workforce Lists (list of employees working on the project from each company); a properly executed Certification of Bidder Regarding Equal Employment Opportunity:; E.O. 11246; a properly executed Contractors Certification Concerning Labor Standards and Prevailing Wage Requirements ; Statement of Compliance and a properly executed Certification of Bidder Regarding Non-Segregated Facilities. Prior to award of this contract, the abovementioned documents must be on file at Department of Community Development within five (5) working days from time of notification. Bidders failure to produce the above certifications in the allotted time, properly signed and executed, or failure to meet the requirements of said certifications shall bar the bidder from being awarded the contract for this Bid. Bid proposals may be held by the Anderson Board of Public Works for a period not to exceed (45) days from the date of the Bid opening for the purpose of reviewing the Bid Proposals and investigating the qualifications of the Bidder prior to the award of a contract. The Board of Public Works reserves the right to reject any and all Bid proposals or to waive any informalities in quoting to the extent permitted by law. One Hundred Percent (100%) of the funds for the undertaking of this project are provided and available under the Community Development Block Grant Agreement between the City of Anderson, Madison County, Indiana and the United States Department of Housing and Urban Development (HUD).
