Main Street Over Indiana Toll Road Preventive Maintenance the Project Consists of the Bridge Deck Overlay and Preventive Maintenance of Bridge No. 301, Main Street Over Itr, Including but Not Limited to Milling and Overlaying the Bridge Deck, Partial and Full Depth Patching to the Concrete Deck, Surface Sealing the Existing Bridge Railings and Copings, Cleaning and Painting Structural Steel and Bearing Assemblies, Filling Voids Under Slope Walls With Flowable Fill, Replacing the Reinforced Concrete Bridge Approach Slabs (Rcbas) and Railing Transitions, Replacing Portions of the Damaged Bridge Rail and Transitions, Replacing the Bridge Deck Longitudinal Expansion Joint Seals, Adding Decorative Signage on the Bridge, Replacing Damaged Bridge Deck Drain System, Concrete Approach Work, Maintenance of Traffic and Replacing the Type I-a Joints. Said Bids Must Be Accompanied With a Certified Check Made Payable to St. Joseph County or Bid Bond Executed by the Bidder and Surety Company in a Sum of Not Less Than Ten Percent [10%] of the Total Amount of the Bid, Together With a Non-collusive Affidavit as Prescribed in Form 96, Revised 2013 The bidder awarded the contract will be required to furnish separate Payment, Performance, and Maintenance Bonds before commencing work. The Payment and Performance Bond shall each remain in effect throughout the entire construction period in an amount equal to the awarded Contract Amount. The required Maintenance Bond shall remain in effect for a period of up to five years from the date of final acceptance in an amount of 10 percent [10%] of the awarded Contract Amount.
