1. INTRODUCTION The County of Calaveras (County), in coordination with Caltrans and the Federal Highway Administration, intends to replace the existing bridge over Youngs Creek on Lime Creek Road. The recommendation is to replace the existing bridge and improve the approach roadway. The replacement bridge will be 57 feet long and 25.5 feet wide and have cantilever wingwalls. The Project will include a new paved approach with side slope orientation and Rock Slope Protection (RSP). The ideal Consultant shall also have experience working with, or in a County or local government agency to provide Resident Engineer, Structure Representative, and Construction Inspector for bridge replacement projects. It is expected that the ideal Consultant shall represent the County and the County's interests at all times while managing the program with minimal guidance from the County. It is the intent of the County to select the most qualified firm to provide the professional services described herein. However, the County reserves the right, at its sole discretion, to terminate the Request for Proposals (RFP) process or negotiation with a selected firm and either perform the work with its staff or begin a new Consultant selection process. Proposers are advised that THIS IS NOT A SEALED BID OR LOW BID PROCESS. The County intends to make an award or awards using the evaluation criteria listed in this specification to determine the professional that will provide the best value for the County. Nevertheless, cost is still a criterion that will be used for evaluation. 2. BACKGROUND AND PURPOSE The Construction Management agreement will be part of the Federal-Aid project Lime Creek Road over Youngs Creek Bridge Replacement Project. Costs or prices based on estimated costs must comply with Federal cost principles. Work is to be completed within a total of 110 working days. The successful firm will provide construction management services. The DBE goal for the Construction Management firm is 13%. The ideal Consultant shall also have experience working with, or in a County or local government agency to provide construction management services associated with the bridge replacement project. It is expected that the ideal Consultant shall represent the County and the County's interests at all times while managing the program with minimal guidance from the County. The project is construction ready. 3. PROJECT BACKGROUND AND SCOPE OF WORK Consultant's scope of work must include performing the duties of Resident Engineer, Structure Representative, and Construction Inspector for all work performed within the Construction of the Lime Creek Road over Youngs Creek Bridge Replacement Project. Performance of duties must be in conformance with Caltrans Construction Manual, other documents referenced in the Caltrans Construction Manual, and procedures for federal-aid projects. Consultant shall be responsible for authorizing or stopping and accepting or rejecting the work of Contractor based on conformance with the construction contract documents including the environmental permits. Duties of Consultant shall include monitoring of Contractor's environmental responsibilities established by the permits and immediately notifying Contractor of any non-compliance with the permits. The Resident Engineer (RE) shall be Consultant's primary point of contact with the County. The RE shall be responsible for preparing all contract change orders, which must be reviewed and approved by County before issuance to Contractor. Consultant shall keep complete project records consistent with the Caltrans Construction Manual and procedures for federal-aid projects. Consultant shall schedule Consultant's work to meet the approved CPM schedule submitted by Contractor. Consultant shall provide all resources necessary to successfully perform the work, including, but not limited to suitable personnel, construction office (if construction office is needed), office equipment, office supplies, office internet and telephone services, field equipment, field supplies, and field telephone service. Consultant shall prepare and submit to Contractor progress payment invoices for signature. Progress payment invoices sign by Contractor shall be submitted to County for processing payment to Contractor. Consultant shall mark-up drawings notating any deviations from the plans. Mark-ups shall be provided to design engineer for completing record drawings. Consultant shall prepare a punch-list near project end. Consultant shall conduct a walk-through with Contractor verifying all punch-list items are satisfactorily completed before acceptance of work. Consultant shall assure the quality of materials and work products by sampling and testing of materials and work products in the field and laboratory, in accordance with the Project Quality Assurance Program and contract specifications. Laboratory test results shall be certified by California Registered Professional Engineer. Consultant shall prepare reports of test results and compliance of materials and work products with the contract specifications. Consultants to be responsible for providing cultural/archaeological activities consistent with the requirements of this project. 4. SERVICES TO BE PROVIDED BY THE COUNTY With the exception of the following items, the Consultant shall provide all necessary resources and services to execute the scope of work summarized in Section 3. In coordination with the selected Consultant, the County will: o provide basic criteria for project design, record drawings, and preprinted portions of the specifications. Consultants are encouraged to review available record information on file at the Calaveras County Department of Public Works offices, 891 Mountain Ranch Road, San Andreas, CA 95249-9709 prior to submitting a proposal. o provide copies of available pertinent County records, such as survey ties, benchmarks, road plans, and record maps the County knowingly has in its possession. o print and distribute final bid documents, unless requested as an additional service. o mediate with land owners for access to and provisions for the Consultant to enter upon private and public lands. o provide all other required services that cannot, by law, be performed by the Consultant. o serve as Contract Administrator for the project. If the Consultant requests the County provide services or information not specifically indicated above, those requests should be clearly stated in its Proposal. 5. PROPOSAL SUBMITTAL INSTRUCTIONS 5.1. Minimum Submittal Requirements: The Proposal shall be limited to specific discussion of and response to the elements outlined in this RFP. The intent of this process is to encourage a response which allows the County to evaluate the respondent's understanding of the scope of the proposed project and experience/qualifications applicable to providing the services required. Complete Submittals shall consist of the following six elements: o Transmittal Letter o Statement of Qualifications o Resumes of Key Personnel o Three References for Key Personnel o Similar Project Experience of Firm and Key Personnel o Technical Proposal o Proposer's understanding of scope of work o Schedule o Organization Chart o Work Plan o Cost Proposal (to be submitted in a separate electronic file) o Licenses and Certifications o Levine Act Statement o DBE Commitment: LAPM Exhibit 10-O1 o Disclosure of Lobbying Activities: LAPM Exhibit: 10-Q o Financial System Certification: LAPM Exhibit 10-K for prime and sub-consultants (to be submitted in a separate sealed envelope) Submittal Requirements: A successful respondent is one whose response complies with all requirements of the RFP. Firms are encouraged to limit proposals to 25 single-sided, 8.5" x 11" pages. This quantity of pages includes the transmittal letter, statement of qualifications and technical proposal. Resumes will not be counted toward the page count. A separate cost proposal shall be submitted as part of the Proposal to Public Purchase. Bid Contact Mark D.
