The work for which bids are being requested consists of the following: o Notify property owners of verification work to be completed on private property both inside and outside of the home or building via door hangers; o Complete interior inspection and installation of bladder tank if required at each property in accordance with the plumbing permit and the project specifications, including identification of material type and completion of the Survey123 electronic questionnaire which includes photos of each verification. The GIS will be accessible after Contract Award. o This project entails modification of existing plumbing and therefore a plumbing permit is required for each installation. Permit applications can be obtained through the Village website A certified check or bank draft drawn on a solvent bank in the State of Michigan payable without condition to the VILLAGE OF MENDON, or a satisfactory bid bond executed by the bidder and a surety company, in an amount equal to 5 percent of the bid shall be submitted with each bid. No bids may be withdrawn after the scheduled closing time for receipt of bids for a period of sixty (60) days to allow for the review of proposals and to allow completion of necessary financial arrangements. The successful bidder will be required to furnish a satisfactory Performance Bond and Payment Bond in the amount of 100 percent of the Contract price. Requests for clarification shall be directed to Frank Renaldi, PE at frenaldi@gowightman.com This project shall consist of contacting property owners and residents to gain access to buildings to which water is supplied and document existing and finished conditions in conjunction with bladder tank installation. Documentation shall include but not be limited to: 1. Hang door tags, set appointments with occupants and document status of each. 2. Obtain access to homes or businesses and follow the prompts on the Survey123 program to fill out the inspection report. 3. Determine whether tank is required and select appropriate size. 4. Take photographs of tank installation location before and after work is complete and link them to the database. 5. Verify and document water service materials.
