The building will be 120'x120' with an interior kitchen and two separate bathrooms. Services include concrete footings and slab, supplying steel structure, erecting steel structure, interior build out including drywall, paint, electrical, plumbing, HVAC, etc. The Paiute Indian Tribe of Utah (Tribe) issues this Request for Proposals ("RFP") from qualified contractors to construct a new steel building at the Tribal Headquarters, located at 440 N Paiute Dr. Cedar City UT 84721. Services include concrete footings and slab, supplying steel structure, erecting steel structure, interior build out including drywall, paint, electrical, plumbing, HVAC, etc.) Scope of Services Bidders are required to meet all criteria outlined in this RFP. A. Pursuant to this RFP, the Services shall include all services to construct the structure. Bid documents included (1) Conceptual rendering (2) Conflict of Interest form and Suspension/Debarment form. Over-excavation will not need to be included in the bid as the Tribe has completed the Over-excavation B. Successful bidders shall have experience in the construction of steel building structures. C. Industry Specific Standards. The successful bidder(s) shall provide all necessary inspections in accordance with the current state and local codes, laws, and ordinances, as well as all rules and regulations of any authorities controlling or limiting the methods and materials to be used or the actions of those engaged in this kind of work, including pertinent health standards. D. Responsibilities. 1. All bidders must provide a signed certification related to Debarment and Suspension of Service, which is included in the Bid Documents. 2. All bidders must disclose and submit the required Conflict of Interest form that is included with the Bid Documents. 3. The successful bidder(s) shall provide all equipment necessary to perform the Services as described in the RFP. The successful bidder(s) shall be licensed to perform the Services and to utilize all equipment necessary to perform the Services as described in the RFP. 4. Contractor shall restore all areas to original condition, including but not limited to the area Services are provided.
